Physical Exam. Executive shall be required, on an annual basis, to undergo a physical examination and to send evidence that Executive has undergone such exam (but in no case the results of such exam) to the Senior Vice President of Organizational Effectiveness. The Company shall reimburse Executive for any out-of-pocket expenses relating to the physical examination that are not otherwise covered by Executive’s health insurance plan.

Physical Exam. 301. SFMTA may require a Operator to pass a return to work fitness for duty exam. For physical examinations, SFMTA may require the Operator to pass a functional capacity exam, and the job description or essential function job analysis for the Operator position will establish the standard for the functional capacity exam. In addition, SFMTA reserves its right under law to require a Operator to successfully pass a fitness for duty exam demonstrating that the Operator can perform the essential functions of the Operator’s position with or without reasonable accommodation.
Physical Exam. During the term of the Executive’s employment, the Company shall reimburse the Executive for all costs associated with Executive’s physical exam at the Cxxxxx Clinic or a similar facility; provided, that the Executive shall only be reimbursed for the costs of one such exam during each calendar year.

Any employee of the Tulare County Office of Education who is required to submit to a physical or mental examination shall have the right to be represented at the time of the examination by a physician or psychiatrist of his or her choice and the report of the employee's physician or psychiatrist shall be filed with the County Superintendent of Schools at the request of the employee. The employee shall, upon request, and before action by the County Superintendent of Schools, be furnished with a complete copy of all reports made by the physician or psychiatrist appointed by the County Superintendent of Schools. When physical or mental examinations are required by the Tulare County Office of Education, one (1) copy of the results shall be sent to the Tulare County Superintendent of Schools. Results of physical and mental examinations shall be kept in a sealed envelope within the employee's personnel file.

Physical Exam. 16.1 If the Superintendent has reason to believe that an administrator’s job performance may be impaired by a physically or emotionally related health problem, the administrator shall be required to undergo a medical examination. Said administrator shall be required to present a signed statement from a mutually agreed upon, duly licensed and qualified physician and/or health care provider attesting to the person’s physical and/or emotional competence to perform his/her contracted job responsibilities. An administrator shall have the right to present any objections to such a request to the Board within 5 days of the initial request. The Board shall either withdraw or reaffirm the request. Failure to comply with such a request shall be grounds for dismissal. All reports from health care providers shall be treated as confidential and shared only with those staff members who, in the judgment of the superintendent, have a need to be informed. Criteria: A lessening of one’s job performance that may be caused by a condition detectable through a medical examination. Indications may include such factors as: a pattern of absences over a period of time; a series of related complaints concerning an administrator’s behavior from students, parents and/or colleagues; and/or a significant and unexplained change in one’s job performance. Records: A report form will be developed indicating 1) if the administrator is medically able to perform one’s duties; 2) if treatment is recommended in order to resolve the performance issue; and 3) if so, is the administrator complying with the treatment recommendation. This report would become part of the administrator’s personnel file only.
Physical Exam. The Board shall provide for a fully paid yearly medical examination for each driver at the medical facility designated by the Board. A driver may elect to have his/her physical exam at a medical facility different than that designated by the Board and such exam will take the place of the exam provided by the Board. The cost of such alternate exam shall be borne by the driver. Notwithstanding the foregoing regarding annual medical exams, the Board reserves the right to have a driver examined by a physician designated by the Board at any time there is any reasonable doubt that the driver is physically or mentally fit to safely operate a bus. The cost of such exam shall be borne by the Board.
Physical Exam. 19.1 Employees covered by this Agreement may be required by the County to undergo an independent medical examination upon return to work from illness or injury in order to determine the employee’s ability to return to work, or if the County has a reasonable suspicion that the employee may suffer from a condition that could affect his or her safety, the safety of others, or his or her ability to perform. The County will bear the cost of such examination. The results of the exam will be made available to the individual employee upon written request.
