Prescription Safety Eyewear. The Employer agrees to reimburse any employee fifty percent (50%) of the cost of prescription safety eyewear up to two hundred and fifty dollars ($250.00) according to the following criteria: The employee must provide a copy of the prescription and receipt for the safety glasses. The employee must have worked twelve hundred (1,200) hours with the Employer for the first reimbursement. For any subsequent reimbursement the employee must have worked an additional four thousand (4,000) hours from the last time reimbursed.
