Radiology Equipment. If the Dental Office utilizes radiology or radiographic equipment at his facility in rendering services pursuant to this Agreement, the Dental Office shall have such equipment regularly checked by local or state health authorities or a radiation physicist to insure that such equipment is environmentally safe and technologically accurate. Any hazards identified by such inspections or at any time shall be promptly corrected. The Dental Office shall maintain equipment maintenance and calibration records and all inspection certificates or reports which shall be available for review by LIBERTY upon request.
Radiology Equipment. If Dental Office utilizes radiology or radiographic equipment at its facility in rendering services pursuant to this Agreement, Dental Office shall have such equipment regularly checked, as required by LIBERTY and applicable laws and regulations, to ensure that such equipment is environmentally safe and technologically accurate. Dental Office shall correct any hazards identified by such inspections or identified at any other time. Dental Office shall maintain equipment maintenance and calibration records and all inspection certificates or reports (collectively, “Equipment Records”) for the time periods specified by law or regulations, and in absence of any applicable law or regulation for a period of ten (10) years from the date of the creation of the Equipment Records. The Equipment Records shall be available for review by LIBERTY upon request.
Radiology Equipment. If Dentist utilizes radiology or radiographic equipment at its facility in rendering services pursuant to this Agreement, Dentist shall have such equipment regularly checked by local or state health authorities or a radiation physicist to ensure that such equipment is environmentally safe and technologically accurate. Any hazards identified by such inspections or at any time shall be promptly corrected. Dentist shall maintain equipment maintenance and calibration records and all inspection certificates or reports, all of which records, certificates and reports shall be available for review by the Plan upon request. In addition, Dentist shall (and shall ensure all applicable Agents) possess and maintain any licenses required to operate such equipment in accordance with applicable laws and regulations, including but not necessarily limited to N.J.S.A. § 26:2D-27.
