RATING PROCEDURES. Performance reports shall be made on forms prescribed by 21 Human Resource Services and following procedures established by Human Resource 22 Services. The rating intervals shall be as indicated below: 24 1. Probationary employees shall be rated at the end of the second, fourth, and sixth 25 month from the date of appointment in that position. 26 27 2. Permanent employees shall be rated annually. The period of the annual rating shall 28 be twelve (12) months following the date the employee gains permanence in his/her 29 present position or twelve (12) months following subsequent regular or revised rating 30 reports, except when the end of the rating period falls during a non-work period for 31 a ten (10) or eleven (11) month employee. Employees will receive a warning and counsel from the rater within ten (10) days of 35 the occurrence of less-than-satisfactory performance. If the less-than-satisfactory 36 performance occurs less than ten (10) days prior to an employee's formal rating, the 37 rater shall warn and counsel the employee prior to the formal rating. The employee 38 may request that a warning be put in writing. The employee may also request the 39 name of the complainant(s) for any report of less-than-satisfactory performance 40 which results in warning and counsel. 41 42 4. Raters shall include a statement of the facts and suggestions for improvement by 43 completing an “Improvement Plan and Strategy for Assistance” form for any rating 44 that includes a "Needs To Improve" (N) or "Unsatisfactory" (U). Content of the 45 comments including those from the “Improvement Plan and Strategy for Assistance” 46 shall not be grievable. 47 1 5. A revised rating may be submitted any time upon evidence of changed work habits 2 or performance on the part of the employee. 3 4 6. Upon satisfactory completion of the “Improvement Plan and Strategy for Assistance” 5 the employee shall be eligible for summer/intersession employment following the 6 assignments of employees whose last fully resolved service rating is satisfactory if 7 additional summer assignments are available. 8
