Reporting Misconduct. Any individual participating in provision of IDEA/Part C services is required to report misconduct to IDEA/Part C State Office within five (5) working days by way of a formal, written complaint (see procedures for Family Rights and Safeguards). If at any point, any individual who reasonably believes that an EIS provider is posing an imminent risk of danger to children, parents, or staff should report the information to a local law enforcement agency or South Carolina Department of Social Services and then to IDEA/Part C State Office within twenty-four (24) hours.
Reporting Misconduct. 163. APD shall require that all officers and employees report misconduct by any APD officer or employee, including themselves, to a supervisor or directly to the Internal Affairs Division for review and investigation. Where alleged misconduct is reported to a supervisor, the supervisor shall immediately document and report this information to the Internal Affairs Division. Failure to report or document alleged misconduct or criminal behavior shall be grounds for discipline, up to and including termination of employment.

EHPD shall require that all officers and employees report apparent misconduct by another EHPD officer or employee to a supervisor or directly to the IAO for review and investigation. Where apparent misconduct is reported to a supervisor, the supervisor shall immediately document and report this information to the IAO. Failure to report or document apparent misconduct or criminal behavior shall be grounds for discipline, up to and including termination of employment. The default discipline for a failure to report criminal behavior shall be termination.

If you feel a Homeowner or Housemate is acting or has acted inappropriately, including but not limited to (i) engaging in offensive, violent or sexually inappropriate behavior, (ii) you suspect of stealing from you, or (iii) engaging in any other disturbing conduct, you must immediately report such person to the appropriate authorities and then promptly (no later than twenty-four (24) hours after reporting to authorities) to Silvernest by contacting us with your police station and report number at xxxxx@xxxxxxxxxx.xxx; provided that your report will not obligate us to take any action beyond that required by law (if any) or cause us to incur any liability to you.
Reporting Misconduct. Church Personnel have a duty to report their own ethical or professional misconduct and the misconduct of others. Church Personnel must hold each other accountable for maintaining the highest ethical and professional standards. When there is an indication of misconduct by Church Personnel, the person who learns of the misconduct should notify his/her supervisor or other appropriate Church authorities.

If you book Transportation Services with a Supplier who you feel is acting or has acted inappropriately, including but not limited to, anyone who (i) engages in offensive, violent or sexually inappropriate behavior, (ii) you suspect of stealing from you, or (iii) engages in any other disturbing conduct, you should immediately report such person to the appropriate authorities and then to Bookaway by contacting us with your police station and report number at xxxxxxx@xxxxxxxx.xxx; provided that your report will not obligate us to take any action beyond that required by law (if any) or cause us to incur any liability to you. Bookaway will not be held liable for any behavior or conduct of any third party, on or around the vehicle, including the behavior of other passengers.

If you use a Mobile Application from a Partner on the ExStreamity LLC platform or find anyone associated with ExStreamity LLC who you feel is acting or has acted inappropriately, including but not limited to anyone who (i) engages in offensive, violent or sexually inappropriate behavior, (ii) you suspect of stealing from you, or (iii) engages in any other disturbing conduct, you should immediately report such person to the appropriate authorities and then to ExStreamity LLC by contacting us with your police station and report number; provided that your report will not obligate us to take any action beyond that required by law (if any) or cause us to incur any liability to you.
Reporting Misconduct. 3.7.1 If any of the following applies in the course of using or in connection with the Service:
(a) you communicate and transact with anyone who you feel is acting or has acted inappropriately, including anyone who engages in offensive, violent, or sexually inappropriate behaviour;
(b) you suspect a person may be attempting to steal from you; or
(c) engages in any other disturbing conduct, you should immediately report such person to the appropriate authorities and then to PetRehomer by contacting us with your police station and crime report number.
3.7.2 Your report will not obligate us to take any action beyond that required by Regulations or cause us to incur any liability to you.
Reporting Misconduct. By signing this agreement, you acknowledge your responsibility to immediately report violations of the UTSA Acceptable Use Policy or these Terms of Use. Report violations immediately to the laboratory assistant and within 24 hours to your instructor.
