Request for Cards. Subject to Bank’s approval upon Organization’s request (e.g., through an Authorized Administrator), Bank will establish Cards linked to Related Accounts for use in making Transactions. Each Card shall be valid through the noted expiration date unless the Bank has canceled such Card or the expiration date has been modified by an Authorized Administrator or Authorized User. Unless the Card is a “Descriptive Title Card” (see below), each Physical Card issued by Bank at the Organization’s request shall bear the Organization’s name as well as the legal name of the natural person who is the Authorized User (such name to be provided to Bank by the Organization through the actions of an Authorized Administrator). When Organization requests a Card, Organization shall provide the name of the Authorized User and other information requested by Bank, such as address, contact and phone information, desired credit limit, etc. for each Card requested. Organization agrees that Bank will mail the requested Physical Card to each Authorized Cardholder at Organization’s business address, unless the Organization furnishes to Bank other instructions. Organization is responsible for ensuring that each Authorized Cardholder to signs any Physical Card issued to him or her. Organization is also responsible for ensuring that Organization’s Authorized Users use each Master Account, Related Account, and Card to which he or she has access in strict accordance with the Agreement. Bank and any merchant or other financial institution may rely upon any individual’s access to or possession of a Master Account, Related Account, or Card as its sole evidence of that the individual is authorized to use it and shall have no duty to question the purpose or nature of that use by such individual.

  • REQUEST FOR REVIEW Within sixty (60) days after receiving notice from the Plan Administrator that a claim has been denied (in part or all of the claim), then claimant (or their duly authorized representative) may file with the Plan Administrator, a written request for a review of the denial of the claim. The claimant (or his duly authorized representative) shall then have the opportunity to submit written comments, documents, records and other information relating to the claim. The Plan Administrator shall also provide the claimant, upon request and free of charge, reasonable access to, and copies of, all documents, records and other information relevant (as defined in applicable ERISA regulations) to the claimant’s claim for benefits.

  • Request for Hearing The employee must file a written request for hearing within fifteen calendar days of receipt of the Notice of Dismissal or Suspension. Filing means receipt in the office designated no later than regular close of business on the last day of the filing period. Failure to file such request in a timely manner shall be deemed a waiver of the right to a hearing and the proposed action shall be effective upon action by the Governing Board without notice or hearing except as may be required in a board meeting agenda.

  • Request for Leave At any time, employees may request the use of short-term leave (annual leave, compensatory time use, or personal leave). Such request shall be submitted on the appropriate form and approved or denied on the form within one week of submission to the appropriate authority (practices of shorter time periods will be maintained) except that current practices concerning emergency leave requests shall be maintained. Requests will not be denied unreasonably. The issue of more employees requesting the use of short-term leave than can be granted because of operational needs shall be resolved at the LMC. The Employer shall not request the reasons for the use of earned leave other than sick leave as permitted by Article 12. All leave may be used in tenth of an hour increments provided, however that use of personal leave to cover tardiness related absences shall not serve as a bar or defense to disciplinary action.

  • Request for Services Customer shall make an appointment with Exasol for Operational Services and coordinate the scope of the services with Exasol. The appointment must be made at least three Business Days for small to medium tasks (less than 0.5 days effort) and 15 Business Days for large tasks (more than 0.5 days effort) before the planned service actions.
