Common use of Requesting Extended Leave Clause in Contracts

Requesting Extended Leave. An employee desiring to utilize an extended unpaid leave under this Section shall file a written request on the specified form with the Personnel Department at least thirty (30) calendar days in advance of the anticipated starting date of the leave. The Director may waive or reduce the thirty (30) day notice.

An A professional employee desiring to utilize an extended unpaid leave under this Section Article shall file a written request on the specified form with the Personnel Department Human Resources at least thirty (30) 30 calendar days in advance of the anticipated starting date of the leave. The Director Superintendent may waive or reduce the thirty (30) 30 day notice. If a professional employee is absent (whether in paid or unpaid status) for 10 consecutive work days, he/she shall complete the specified leave form and return it to the immediate supervisor who will forward the form to Human Resources.

If an employee is absent (whether in paid or unpaid status) for ten (10) consecutive work days, he/she shall complete the specified leave form and return to the immediate supervisor who will forward the form to the Personnel Division.
