Extended Disability Leave a. Due to medical disability, an employee shall be granted an extended leave of absence without pay (subject of Paragraph d hereof) if any one of the following conditions exists:
(1) A notable and substantial decrease in work performance.
(2) The production of a certification from a medical doctor that the employee is medically unable to continue to work.
b. The Board shall have the right to have such an employee examined by its own physician and in the event of a disagreement between the Board's physician and the employee's physician on such ability or inability, the question shall be referred to a third physician designated by mutual agreement of the employee and the Board, or, if no such agreement can be reached, by the Camden County Medical Society, for final and binding resolution. The expenses of the third physician shall be paid by the Board.
c. During the period of the employee's personal medical disability, accumulated sick leave benefits in accordance with "SICK LEAVE - ARTICLE VII" shall be paid until such benefits are exhausted or the personal medical disability has terminated.
d. When the seeking of an extended leave of absence for medical disability can be anticipated, an employee shall file a written request for such leave with the Superintendent or his designee at least thirty (30) days in advance of the anticipated date on which said leave is to commence. The request shall specify therein the date upon which, subject, to medical confirmation, the employee proposes to return. Upon termination of the medical disability, the employee shall return to work, resign, retire or apply for other leave. In the event of disagreement, the date of such termination shall be established as set forth in Paragraph b above.
e. Application for extended medical disability shall not be granted beyond June 30 of the current school year. In the event an employee wishes to continue on extended medical disability for all or part of the ensuing school year, an application for such status shall be made during the month of June accompanied by appropriate medical certification and shall be granted by the Board. In the event the continued extended medical disability is for part of the ensuing school year and there is a change in the employee's medical status during the school year, the Board shall, upon submission of an additional application and appropriate medical certification, make changes in said leave. Extended Disability Leave. If the employee is still disabled at the conclusion of all available paid disability leave(s) and is unable to return to work, Article 12, Section 2, Extended Disability Leave, shall apply.
Extended Disability Leave. 3.1 When a member of the bargaining unit is absent from duty on account of illness or accident for a period of five calendar months or less, whether or not the absence arises out of or in the course of employment of the employee, the amount deducted from the salary due the employee for any month in which the absence occurs shall not exceed the sum which is actually paid a substitute employee employed to fill the absent employee's position during the absence.
3.2 To be eligible for leave pursuant to this Section, unit members must submit doctor's verification of illness to the Human Resources Department on forms provided by the District
3.3 The leave and differential salary provided in subsection 3.1 above is available only after all accumulated sick leave and earned vacation have been exhausted.
3.4 Extended disability is defined as disability of long, continued, and indefinite duration which prevents an employee from performing his/her usual duties. Extended disability leave is not intended to be used for illness of short duration.
3.5 In case of illness of short duration--five days or less-- where all available sick leave has been exhausted, the employee may use leave without pay or vacation leave.
3.6 Extended disability leave is available only once in each fiscal year. The five-calendar-month period is calculated commencing with the first day of absence for the illness defined as qualifying the unit member for the leave provided in this section.
Extended Disability Leave. 373 The University provides an interim income protection plan for regular full-time staff with at least sixty (60) continuous FTE service months and who are expected to receive long term disability benefits. The benefit provides full pay as defined in the long term disability master contract, for a period of up to one hundred eighty (180) days from the Employee's last day of active work. The benefit commences upon the expiration of all accrued sick, vacation, and personal leave. -374 Determination of whether a case is expected to result in total disability can sometimes be made by the University designated physician based on information received from the Employee's physician and other medical reports. A lack of conclusive medical evidence may delay approval of the extended disability leave on a retroactive basis. In these cases, it is necessary for the Employee to maintain benefit payments. Reimbursement will be made for University contribution amounts upon approval for Extended Disability Leave. -375 Employees expected to return to work within one hundred eighty (180) days will not qualify for this benefit. Authorization to pay extended disability leave benefits will be made by the Benefits Office. The University pays the cost of the benefit. No application is necessary for enrollment. The benefit becomes effective upon completion of sixty (60) full-time equivalent service months. The benefit terminates on the last day of employment. Conversion of coverage is not possible. -376 The Association acknowledges that an Employee may enter into repayment agreement with the University requiring the Employee to repay the University all Extended Disability Leave benefits paid if the Employee does not receive Long-Term Disability benefits. Such agreements shall include a statement that Employee, at their option, may consult with Association or other representative prior to signing this Agreement.
Extended Disability Leave. A. Regular full-time employees with at least sixty (60) continuous FTE service months are eligible for extended disability leaves.
Extended Disability Leave. Upon exhaustion of all accumulated sick leave, a unit member who would otherwise qualify for sick leave under the provisions of this Article will receive up to one hundred (100) days of Extended Disability Leave. A unit member on Extended Disability Leave will be paid their regular daily rate of pay minus the Board of Education adopted rate of pay for a substitute per day but no less than fifty percent (50%) of the unit member’s regular daily rate of pay. Unit members on Extended Disability Leave will furnish a release from a licensed health care practitioner before being allowed to return to work.
Extended Disability Leave a. An employee may request an unpaid leave of up to one (1) year for medical reasons. In the event the leave is used for childcare, that portion of the leave days during which the employee is disabled because of childbirth may be charged against the employee’s accrued leave days.
b. An employee who is unable to work because of personal illness or disability and who has exhausted all leave available may be granted a leave of absence without pay for the duration of such illness or disability up to one (1) year. The employee may request extension of the unpaid leave beyond one (1) year.
Extended Disability Leave. 1. Extended disability leave shall be granted after one (l) year of employment upon the approval of the employee’s application for benefits by the insurance company according to the provisions of the group insurance policy issued by the insurance company to PEF. This benefit shall be provided at no premium cost to employees with a maximum benefit payment at the 60% benefit level, equal to the level of benefits provided to employees on June 30, 1999. The pre-existing condition clause shall be waived for all employees in the unit as of the date of execution of this Agreement. In the event that the employee has not exhausted all of his/her earned leave accruals the employee may choose to freeze their accruals for the duration of the extended disability leave or to use the remaining leave accruals. The use of accruals will be concurrent with the approved extended disability leave. If the employee chooses to freeze the accruals and does not return to work after two years as defined in Article 15 (B) (3) (b) (9), the employee may use any unused sick leave as a funding source for payment of health insurance under COBRA. The employee shall be compensated for any unused vacation leave up to the maximum of 40 days as provided in Article 14 (E)(3).
2. Extended disability leave shall be a leave that may combine the use of any unused accruals, an unpaid leave from PEF and the long term disability insurance program, the duration of which cannot exceed a maximum of 18 months.
3. In order to insure the continuation of benefits under this agreement, the employee should apply to the insurance company for the long term disability insurance benefits they may be eligible for while they are on PEF’s payroll, at least two (2) months prior to the expiration of their extended sick leave benefits, with a copy to PEF.
4. Employees approved for the extended disability leave will have exclusive hold rights to their position and remain fully covered under the provisions of the employee health insurance benefits for a period of two (2) years from the date that the employee commenced his/her absence of 2 continuous years due to a non-job related prolonged illness or temporary physical or medical disability or two and one half (2 1/2) years from the date that the employee commenced his/her absence of 2 1/2 continuous years due to a job related prolonged illness or temporary physical or medical disability.

Extended Disability Leave. An employee who exhausts accumulated annual leave for a disability may apply for shared leave (see Section
6.01 C). If the employee exhausts both annual leave and any shared leave, an extended disability leave without pay and benefits shall be granted for the duration of the disability or through the school year following the exhaustion of annual and shared leave, whichever occurs sooner. The District shall have the discretion to grant additional extended disability leave based on the employee's prognosis for recovery.
