Required Approval and Certification of Illness and Injury. The use of paid 13 illness/injury leave is subject to the approval of the Department Head or designated 14 representative. Such leave may be permitted without requiring the employee to 15 submit a doctor's certificate provided the Department Head has other satisfactory 16 evidence of bona fide illness or injury. However, when such leave extends beyond 17 three consecutive working days, a statement from the attending physician or other 18 qualified medical professional, certifying to the nature and seriousness of the illness 19 or injury, with or without limitations and if the former, to specify the limitations, 20 shall be furnished to the Department Head. Employees who use such leave may be 21 required, upon return to work, to file a written report on a form furnished by the City, 22 stating the length of absence and nature and effect of the employee's illness or injury.
