Revised Laboratory Requisition Form. The Parties agree that there is overuse/misuse of certain laboratory tests. As such, it was recommended that these laboratory tests be removed from the Ontario laboratory requisition form but still available to patients with appropriate indications. They were removed effective November 1, 2012: □ Removal of Ferritin; □ Thyroid stimulating hormone; and □ Vitamin B12.

  • DRUG-FREE WORKPLACE FORM The Drug-Free Workplace Form is attached and shall be completed and submitted with your bid.

  • Drug-Free Workplace Certification As required by Executive Order No. 90-5 dated April 12, 1990, issued by the Governor of Indiana, the Contractor hereby covenants and agrees to make a good faith effort to provide and maintain a drug-free workplace.
