The notice to CONTRACTOR shall specify the date of delivery 30 and collection of the Roll-Off Containers, the location(s) for delivery, and the number 31 and size of the Roll-Off Containers to be delivered; provided, however, that the City 32 Representative may request reasonable changes in the number or size of Roll-Off 33 Containers up to and including the day of the clean-up event. Each clean-up event 34 shall consist of a single collection day beginning at 6:00 a.m. and ending at 6:00 35 p.m.

Each Targeted Clean-Up event shall consist of a single collection day, Monday through Saturday, beginning at 8:00 a.m. and ending no later than 6:00 p.m. Next day collection may be requested or approved by the City Representative. a) Collection or delivery to or from school, activity, or Outside Service Provider: by Dakoda Xxxxxxxx, Xxxx Xxxxxxx or Xxxxxxx Xxxxxxxx, as agreed and/or required. NOTE: Benefit Risk Excursion and Hazard Management Audit forms are to be signed by the Parent/Guardian where applicable, in the case of an emergency/unforeseen circumstance, where applicable - authorisation can be made via written text/email and accepted as signed by the Provider. A copy of same is to be kept in the Participants Portfolio under Authorisations.

SCHEDULE OF SERVICE. ▪ The initial term of this contract is twelve (12) months. The District reserves the option to extend the contract for up to two (2) additional twelve (12) month periods upon the same terms and conditions as provided in the contract. The Contractor can propose a price increase if desired. The Contractor should include a cost schedule for Year 1, 2 and 3 on the Lump Sum Bid Form (Exhibit “B”).

NOTE: Kat’s Care Services (Xxxxxxx Xxxxxxxx) can automatically claim annual indexed price limits without the need to update the existing Service Agreement.
c) Food is included in daily rates as per the NDIS Price Controls (most recent version) – however specific dietary requirements will need to be documented as per Doctor advise for allergies/sensitivities, please complete a Medical Condition Management Plan.
