SELF-SCREENING. Bargaining unit employees shall be responsible for self-screening and reporting whether they have COVID-19 symptoms to their Program Director or were in close contact with someone who tested positive. Bargaining unit employees shall not come into work if ill or if in close contact with someone who tested positive. In such cases, bargaining unit employees shall self-quarantine in accordance with the guidance from the CDC and DESE. The EMPLOYER may request employees to be tested at the EMPLOYER’s expense. Bargaining unit employees will be expected to work-from-home, if possible, during these circumstances, unless they are unable to due to their health.

SELF-SCREENING. Engage in a self-screening assessment meeting the then current requirements and recommendations of the CDC, OSHA, Cal/OSHA, and other applicable state or local public health authorities, each day no more than 2 hours prior to entering any Premises (“Daily Self Screening”). For Representatives who require COVID-19 testing as described herein, each individual who has access to a mobile device should conduct the Daily Self Screening by using FOX’s WorkMatters Guest Daily Screening tool located here: xxxxx://xxxxxxxxxxx.xxxxxxxx.xxx/daily-guest prior to arrival each day they intend to access any Premises. If they receive a green screen and also meet FOX’s COVID-19 testing requirements, they are cleared to enter FOX Premises that day. If such an individual requiring COVID-19 testing hereunder does not have access to a mobile device, they may be required to undergo an in-person Daily Self Screening conducted by FOX (or its designee), which is available at entry points to certain Premises, including the FOX Studio Lot in Los Angeles, CA, and at 0000 Xxxxxx xx xxx Xxxxxxxx xx Xxx Xxxx, XX. Otherwise, they should conduct a Daily Self Screening each day no more than 2 hours prior to entering any Premises. By permitting any Representative to enter any Premises, Renter represents and warrants to FOX that Renter has received confirmation from such Representatives that they have undergone a Daily Self Screening for the applicable day and based on the results, no symptoms/exposures were identified and the person is permitted to access FOX Premises. Renter agrees to regularly review and be fully compliant with all applicable state and local laws and regulations, public health department directives, and CDC, OSHA, and Cal/OSHA guidelines relative to COVID-19.

Such symptoms can be found in the Canton Public Schools Protocols for Responding to COVID-19 Scenarios, which can be found at xxx.xxxxxxxx.xxx; They are incorporated herein by reference, with the understanding that they may be updated appropriately in accordance with guidance from state and local officials. In the event of such symptoms, the employee should not report to work and should contact a healthcare professional for additional directions on testing, isolation, self-quarantine and/or alternative diagnosis. The Employer shall reimburse the employee of the cost of such test to the extent not covered by the employee’s health insurance or other program. Employees shall quarantine as provided in DESE protocols found at Canton Public Schools Protocols for Responding to COVID-19 Scenarios.

Bargaining unit employees shall not come into work if ill or immediately report to their supervisor if symptoms arise after their arrival. In such cases, employees will stay home for a duration in accordance with CDC guidelines or a negative test result. The EMPLOYER may request employees to be tested at the EMPLOYER’s expense. Bargaining unit employees may be able to work-from-home during these circumstances or use accumulated sick leave.

  • Health Screening The Contractor shall conduct a Health Needs Screen (HNS) for new members that enroll in the Contractor’s plan. The HNS will be used to identify the member’s physical and/or behavioral health care needs, special health care needs, as well as the need for disease management, care management and/or case management services set forth in Section 3.8. The HNS may be conducted in person, by phone, online or by mail. The Contractor shall use the standard health screening tool developed by OMPP, i.e., the Health Needs Screening Tool, but is permitted to supplement the OMPP Health Needs Screening Tool with additional questions developed by the Contractor. Any additions to the OMPP Health Needs Screening Tool shall be approved by OMPP. The HNS shall be conducted within ninety (90) calendar days of the Contractor’s receipt of a new member’s fully eligible file from the State. The Contractor is encouraged to conduct the HNS at the same time it assists the member in making a PMP selection. The Contractor shall also be required to conduct a subsequent health screening or comprehensive health assessment if a member’s health care status is determined to have changed since the original screening, such as evidence of overutilization of health care services as identified through such methods as claims review. Non-clinical staff may conduct the HNS. The results of the HNS shall be transferred to OMPP in the form and manner set forth by OMPP. As part of this contract, the Contractor shall not be required to conduct HNS for members enrolled in the Contractor’s plan prior to January 1, 2017 unless a change in the member’s health care status indicates the need to conduct a health screening. For purposes of the HNS requirement, new members are defined as members that have not been enrolled in the Contractor’s plan in the previous twelve (12) months. Data from the HNS or NOP form, current medications and self-reported medical conditions will be used to develop stratification levels for members in Hoosier Healthwise. The Contractor may use its own proprietary stratification methodology to determine which members should be referred to specific care coordination services ranging from disease management to complex case management. OMPP shall apply its own stratification methodology which may, in future years, be used to link stratification level to the per member per month capitation rate. The initial HNS shall be followed by a detailed Comprehensive Health Assessment Tool (CHAT) by a health care professional when a member is identified through the HNS as having a special health care need, as set forth in Section 4.2.4, or when there is a need to follow up on problem areas found in the initial HNS. The detailed CHAT may include, but is not limited to, discussion with the member, a review of the member’s claims history and/or contact with the member’s family or health care providers. These interactions shall be documented and shall be available for review by OMPP. The Contractor shall keep up-to-date records of all members found to have special health care needs based on the initial screening, including documentation of the follow-up detailed CHAT and contacts with the member, their family or health care providers.

  • Message Screening 9.4.4.1 BellSouth shall set message screening parameters so as to accept valid messages from Global Connection local or tandem switching systems destined to any signaling point within BellSouth’s SS7 network where the Global Connection switching system has a valid signaling relationship.

  • Background Screening VENDOR shall comply with all requirements of Sections 1012.32 and 1012.465, Florida Statutes, and all of its personnel who (1) are to be permitted access to school grounds when students are present, (2) will have direct contact with students, or (3) have access or control of school funds, will successfully complete the background screening required by the referenced statutes and meet the standards established by the statutes. This background screening will be conducted by SBBC in advance of VENDOR or its personnel providing any services under the conditions described in the previous sentence. VENDOR shall bear the cost of acquiring the background screening required by Section 1012.32, Florida Statutes, and any fee imposed by the Florida Department of Law Enforcement to maintain the fingerprints provided with respect to VENDOR and its personnel. The parties agree that the failure of VENDOR to perform any of the duties described in this section shall constitute a material breach of this Agreement entitling SBBC to terminate immediately with no further responsibilities or duties to perform under this Agreement. VENDOR agrees to indemnify and hold harmless SBBC, its officers and employees from any liability in the form of physical or mental injury, death or property damage resulting from VENDOR’s failure to comply with the requirements of this section or with Sections 1012.32 and 1012.465, Florida Statutes.
