Services of Other Providers. The services of outside providers such as a licensed physician, psychologist and dentist, a registered pharmacist and licensed pharmacy for the provision of pharmaceutical supplies, a licensed hospital, rehabilitation therapies and diagnostic services, laboratory, x-ray, podiatry, optometry, medications, ambulance services and hearing aid repair are available from time-to-time at the Facility. These services are available under guidelines and procedures established by the Facility and may be utilized by Resident at his or her own expense. For a resident whose payor source is Medicare Part A or Medicaid, the charges related to those services which are covered under the programs will be billed to Medicare Part A or Medicaid accordingly.

Services of Other Providers. The Resident may receive services from outside providers in the Community so long as the outside provider is properly licensed and certified under the law, complies with all applicable government rules and the Community policies. You must notify the Community prior to engaging any outside person or entity to provide services to Resident at the Community in order to discover what particular rules and policies may apply.
Services of Other Providers. You agree to be responsible for charges incurred by outside providers, and not to hold the Community liable in any way for the cost of those services.
Services of Other Providers. Third-party healthcare providers (e.g. physicians, hospice, medical supplies, pharmacy, temporary nursing care, laboratory, X-ray, physical therapy, occupational therapy, speech pathology, podiatry, dental, optometry and hearing aid repair) may make their services available to you on The Community’s premises from time to time upon your request or by our invitation. You have the right to directly engage any licensed healthcare provider or private duty provider of your choice, so long as you comply with any applicable state or other governmental regulations governing skilled care services, and so long as neither you nor the provider, nor the provision of such services, is disruptive to The Community or other residents of The Community. Without limiting the foregoing, such licensed healthcare provider or private duty provider shall not dine in the Community’s dining rooms. These services will be billed directly by the provider to you or your third-party payer, if applicable. You agree to take the full legal responsibility for any injury, damage or loss caused by any provider of such services to you, your property or to others or their property including, but not limited to, us, other residents of The Community and their employees, guests and contractors. You agree that we are not legally responsible to you or any other person for such injury, damage or loss. You agree that we may exclude persons who provide any personal care, other services or goods to you on the premises of The Community for reasonable cause, including, but not limited to, their failure to comply with our rules for the conduct of non-residents on the premises of The Community, or evidence of past misconduct involving abuse, neglect or financial exploitation of the elderly.

However, these fees and costs are not included in the Basic Daily Rate and may be charged directly to the Resident by the outside provider. Resident is responsible for payment of these charges. Resident may choose to utilize providers of his or her own choice; however, the services and goods provided must meet the standards established by Facility.

These ordered Network Elements and Other Services manual additive NRCs will apply in these states, rather than the charge per LSR. The per-element charges are listed on the Rate Tables in Exhibit C.

  • Related Services Licensee shall be responsible for obtaining and installing all proper hardware and support software (including operating systems) and for proper installation and implementation of and training concerning the Licensed Software.
