Single Life Insurance. Based upon the premiums for a $15,000 group term life insurance policy (“Life Insurance Premiums”), the Board shall contribute 100% of the Life Insurance Premiums on behalf of each eligible DFSA member.

Single Life Insurance. Basic life insurance coverage in the principal amount of $30,000 shall be provided for each eligible teacher subject to the terms of the policy or plan in force. The premiums for this life insurance policy shall be paid in full by the Board. Additional life insurance shall also be made available at the expense of the member subject to the terms of the policy or plan in force.

  • Supplemental Life Insurance In addition to the life insurance benefits provided by this agreement, employees may subscribe voluntarily and at their own expense for supplemental life insurance. Employees may subscribe for an amount not to exceed five hundred thousand dollars ($500,000), of which one hundred thousand ($100,000) is a guaranteed issue, provided the election is made within the required enrollment periods.

  • Group Life Insurance Plan Section 1 - Eligibility Regular full-time and regular part-time employees who are on staff January 1, 1979 or who join the staff following this date shall, upon completion of the three-month probationary period, become members of the Group Life Insurance Plan as a condition of employment.

  • HEALTH AND LIFE INSURANCE The Sheriff will continue to offer to employees of this bargaining unit the same health and life insurance programs being offered to other Sheriff’s Office employees. Due to the changing nature of the health insurance market, the Sheriff retains the right during the term of this Agreement to develop plan changes or to change carriers in order to reduce costs or for other reasons. Changes to the level and types of benefits shall be subject to bargaining as provided by law. Rates paid by the employee will be established by the Sheriff.
