Sleep Studies. 1. Coverage is provided for electro-diagnostic tests used to diagnose sleep disorders, including obstructive sleep apnea. These tests may also be used to help adjust a treatment plan for a sleep disorder that has been previously diagnosed. These tests may be done at home, freestanding facilities, outpatient hospital facilities, or at a sleep disorder unit within a hospital. 2. Prior authorization is required for facility-based sleep tests, independent sleep clinic services, and inpatient sleep tests. Prior authorization is not required for home sleep tests.
Sleep Studies. The Plan provides Benefits for Medically Necessary sleep studies. The Benefit is limited to a maximum of two sleep studies per Calendar Year. Home-based sleep studies may save you money over facility-based sleep studies. Ask your Provider if a home-based sleep study is an appropriate option for you. Call Member Services at 855-624-6463 Monday-Friday, 8am-6pm, if you need assistance finding a network home sleep study Provider.
Sleep Studies. If an employee is required to perform a sleep study pertaining to their CDL, the employee will receive the next work day off. The day off will be compensated at regular time.
Sleep Studies. I. V. medication and pain medication are covered service expenses to the extent they would have been covered service expenses during an inpatient hospital stay. At our option, we may authorize the purchase of the equipment in lieu of its rental if the rental price is projected to exceed the equipment purchase price, but only from a provider that we authorize before the purchase.
Sleep Studies. The professional fees for sleep studies shall be reduced by 5%.
