Clinic Services. The clinic services available at Memorial as of September 10, 2014 will continue to be available in the ▇▇. ▇▇▇▇▇▇ ▇. Garcia – Memorial Family Health Center. These include the current and the expanded services listed below: a. Adequate and appropriately furnished and equipped space within the ▇▇. ▇▇▇▇▇▇ ▇. Garcia-Memorial Family Health Center for twelve (12) District enrollment counselors and one (1) receptionist. The District and ▇▇▇▇▇ also agree to evaluate the future need for such space for District enrollment counselors and, if agreed to, modify such space in the future, including potentially providing space within Shoreline for District enrollment counselors; b. Extended ▇▇. ▇▇▇▇▇▇ ▇. Garcia-Memorial Family Health Center hours to include operating 24 hours on Thursday, Friday and Saturday for non- scheduled visits, with future adjustments to such hours of operation based on patient demand and community need, including potentially operating 24 hours in the health center on Sunday, Monday, Tuesday, and Wednesday in addition to Thursday, Friday, and Saturday; provided ▇▇▇▇▇ will not reduce the number of days the ▇▇. ▇▇▇▇▇▇ ▇. Garcia- Memorial Family Health Center operates 24 hours for at least six (6) months; c. Faculty and residency clinic focused on primary care services (family medicine); d. Specialty clinics, including urology, cardiology, neurology, endocrinology, orthopedics, and post-trauma care; e. Clinic outpatient pharmacy services and medication counseling education; f. X-ray and laboratory services; g. Social services; h. Community health/transition care workers to assist patients and families in navigating needed healthcare services; i. Behavioral health counseling; j. Wellness and prevention education for both patient and families, including help with appropriate diet and lifestyle; k. Management of ongoing diseases such as diabetes, heart conditions, and high blood pressure; and l. Support of spiritual needs through availability of an onsite prayer room.
Clinic Services. The Clinic Services shall include each Service Offering, and the corresponding Performance Measures and Clinic Service Fee listed in Table II below. eScreen shall pay Clinic the fees set forth in the table below. The Clinic Services listed in Table II below are provided to eScreen Customers by Clinic.
Clinic Services. In addition to the services described in this Exhibit B and the Agreement which are applicable to the Clinics, Vendor shall provide as "Clinic Services" the Services which are identified on the service matrix set forth on SCHEDULE C. Descriptions of the Services identified on SCHEDULE C are attached as Schedule 2 to this Exhibit B.
Clinic Services. Hospital shall pay to Contractor per Work Relative Value Unit (as defined by the Centers for Medicare & Medicaid Services from time to time) (“wRVU”) for those Professional Services rendered to Clinic Patients by Contractor, provided, however, that Contractor is in compliance with the terms and conditions of this Agreement and applicable Laws. (a) Contractor shall be paid on the CPT codes submitted to and verified by Hospital professional billing office coders. (b) Contractor must complete clinical documentation using Hospital’s system on all submitted claims prior to reimbursement. (c) Compensation will be calculated as follows: (i) Thirty-Six Dollars and Forty Cents ($36.40) per wRVU personally performed by a Group Physician, or (ii) Forty-One Dollars and Eighty-Six Cents ($41.86) per wRVU personally performed by a Group Physician during such time when the Group Physician is performing such services while directly supervising a family medicine resident at the request of Hospital.

Clinic Services. ADPI Parties shall provide Clinic Services to the PDG Offices through the Closing Date, and, with respect to each PDG Doctor practicing in a PDHC Office, ADPI Parties shall provide Clinic Services until the earlier of Doctor Transition Date or the date on which such PDG Doctor is no longer practicing in that PDHC Office. Effective as of the Closing Date, the PDG Parties shall employ all of their own Clinic-based staff within the PDG Offices, and ADPI Parties will no longer provide any Clinic Services with respect to the PDG Offices. However, ADPI Parties will continue to provide Clinic Service to each PDG Doctor in each Clinic which is a PDHC Office until the earlier of the Doctor Transition Date or the date on which that PDG Doctor is no longer practicing in that Clinic.
Clinic Services. In recognition of the mutual obligations of the Parties hereunder, Hospital and Contactor acknowledge that there shall be no monetary compensation to Contactor for the Clinic Services furnished by Contractor hereunder.
Clinic Services. Employees may utilize non-surgical clinical services provided by the Employer on a walk-in basis. The office visit is provided at no cost, but the employee is responsible for the cost of any medically indicated supplies, medications, devices and tests that require outside lab work. Non-surgical services include insertion and removal of IUDs, diaphragm fittings, ultrasounds, and like procedures but do not include colposcopies, biopsies, MABs and TABs, LEEPs, and sterilizations.
