Termination of Sickness Disability Program. 9 Thirty (30) days from the date a physician determines that an employee is 10 permanently and totally disabled, or that the employee will never return to 11 unrestricted duty within the Police service, the employee will make 12 application for disability retirement benefits under either or both Section 13 40.63 or 40.65, Wisconsin Statutes. Thirty (30) days after certification of 14 disability, or within thirty (30) days after receipt of the decision of the 15 Worker's Compensation Division, DWD, determining the employee 16 eligible for benefits under Section 40.65, Wisconsin Statutes sickness 17 disability benefits will cease. Employment will simultaneously terminate, 18 subject to existing rules regarding use of accrued vacation days. The City 19 will provide all appropriate assistance to employees who decide to make 20 application for disability retirement.

  • Long Term Disability (LTD 4.7.1 The school board shall cooperate in the administration of the LTD Plan. It is understood that administration means that the school board will co-operate with the enrolment and deduction of premiums and provide available necessary data to the insurer, upon request. The school board will remit premiums collected to the carrier on behalf of the teachers.

  • Illness or Disability If, because of Employee’s illness or other disability for a continuous period of more than 45 days, Employee is unable to render the services required by the Company as provided herein, the Company may terminate Employee’s employment hereunder by written notice to Employee at least 30 days in advance of such termination. Upon such termination Employee shall not be entitled to any further payments of any nature, except for payment of (a) any earned but unpaid Annual Base Salary, (b) any unpaid bonuses and (c) unreimbursed business expenses (collectively, “Payable Amounts”).   • Death or Disability The Executive's employment shall terminate automatically upon the Executive's death during the Employment Period. If the Company determines in good faith that the Disability of the Executive has occurred during the Employment Period (pursuant to the definition of Disability set forth below), it may give to the Executive written notice in accordance with Section 12(b) of this Agreement of its intention to terminate the Executive's employment. In such event, the Executive's employment with the Company shall terminate effective on the 30th day after receipt of such notice by the Executive (the "Disability Effective Date"), provided that, within the 30 days after such receipt, the Executive shall not have returned to full-time performance of the Executive's duties. For purposes of this Agreement, "Disability" shall mean the absence of the Executive from the Executive's duties with the Company on a full-time basis for 180 consecutive business days as a result of incapacity due to mental or physical illness which is determined to be total and permanent by a physician selected by the Company or its insurers and acceptable to the Executive or the Executive's legal representative.

  • Personal Illness or Disability The bargaining unit member may use all or any portion of his leave to recover from his/her own illness or disability.

  • Expiration of Sick Leave Credits Employees who are absent due to sickness beyond their accumulated sick leave credits shall be placed on an unpaid leave of absence until they are in receipt of long-term disability benefits.   • Disability If, as a result of the Executive's incapacity due to physical or mental illness, the Executive shall have been absent from his duties with the Company on a full-time basis for six months and within 30 days after written notice of termination is thereafter given by the Company the Executive shall not have returned to the full-time performance of the Executive's duties, the Company may terminate this Agreement for "Disability."
