UNION DUES AND ASSESSMENTS. L401 On (a) The Employer shall, as a condition of employment, deduct from the wages or salary of each pay date on which an employee in the bargaining unit, whether or not the employee is paid a member of the Board Union, the amount of the regular dues payable to the Union by a member of the Union. (b) The Employer shall deduct from each Occasional Teacher any employee who is a member of the ETFO dues and Union any dues chargeable by the Local or an equivalent amount. The amounts shall be determined by ETFO and/or Local assessments levied in accordance with their respective constitutions the Union constitution and forwarded in writing (or) bylaws and owing by the employee to the Board at least thirty Union. (30c) days prior to Deductions shall be made for each biweekly payroll period and membership dues or payments in lieu thereof shall be considered as owing in the expected date of change. L402 The ETFO dues deducted in L401 period for which they are so deducted. (d) All deductions shall be remitted to the General Secretary President of ETFO the Union no later than twenty-eight (28) days after the fifteenth date of deduction and the Employer shall also provide the following information with the remittance: • Social Insurance Number; • Surname and First Name; • Job Classification Number and Job Step; • Gross pay; • Month-to-Date Dues. For new hires, the Union will be provided with the name, sex and address in addition to the above. (e) Before the Employer is obliged to deduct any amount under (a) or (b) above, the Union must advise the Employer in writing of the month following amount of its regular dues. The amount so advised shall continue to be the amount to be deducted until changed by further written notice to the Employer signed by the President of the Union. Upon receipt of such notice, such changed amount shall be the amount deducted. (f) From the date on which of the signing of this Agreement and for its duration, no employee organization other than the Union shall be permitted to have membership dues or other moneys deducted by the Employer from the pay of the employees in the bargaining unit. (g) The Employer shall supply each employee, without charge, a receipt for income tax purposes in the amount of the deductions were madepaid to the Union by the employee in the previous year. Such remittance receipts shall be accompanied provided to the employees prior to March 1 of the succeeding year. (h) An employee shall, as a condition of continued employment, complete an authorization form providing for the deduction from an employee's wages or salary the amount of the regular dues payable to the Union by a list showing the names, addresses, wages earned, days worked, wages for the period, amounts deducted, FTE, Ministry Identification Number (MIDENT) and OCT number. The Board shall provide the information in electronic format.
