Common use of Vision Care Benefits Clause in Contracts

Vision Care Benefits. A-8.01 All eligible employees shall be entitled to vision care benefits for themselves and their eligible dependants. Vision care benefits shall cover one hundred (100%) percent of the cost of eyeglasses and/or contact lenses and/or eye exam appointments to a maximum of two hundred and fifty ($250.00) dollars per employee and two hundred and fifty ($250.00) dollars per each eligible dependent every two (2) years.

Vision Care Benefits. A-8.01 B-6.01 All eligible non-probationary employees shall be entitled to vision care benefits for themselves and their eligible dependantsdependents. Vision care benefits shall cover one hundred percent (100%) percent of the cost of eyeglasses and/or contact lenses and/or eye exam appointments to a maximum of two hundred and fifty dollars ($250.00) per person every two (2) years and one-hundred percent (100%) of the cost of eye exam appointments to a maximum of one hundred dollars per employee and two hundred and fifty ($250.00100.00) dollars per each eligible dependent person every two (2) years.
