Vision Care Sample Clauses

Vision Care. The City agrees to maintain current employee and dependent coverage in the established City vision care plan. For the purpose of this provision, dependents shall include domestic partners of represented employees who have filed a Declaration of Domestic Partnership in accordance with established City policy. The City shall meet and confer regarding any reduction in benefits to the extent it is within the mandatory scope of bargaining required by state law.
Vision Care. Eligible employees shall be provided with vision care coverage allowing for one pair of eyeglasses or contact lenses up to two- hundred dollars ($200) in a twenty-four (24) month period. Vision care benefits will be paid for eligible employees, their spouses and eligible children, provided that they are not enrolled in another comparable plan. Reimbursement shall be provided upon provision of prescription confirmation and original purchase receipt.
Vision Care. Effective July 1, 2000, the District shall provide all full-time employees and their dependents with Vision Service Plan (VSP) Plan C. This plan shall provide for a comprehensive exam and new lenses every 12 months, and new frames every 12 months. All other services will be pursuant to the standard VSP plan description, except that it will reimburse up to $50 for examinations by non-panel providers. There shall be a $10 annual deductible on materials only. In addition, the following vision plan enhancements shall take place effective July 1, 2000: $60 wholesale frame allowance; computer glasses; progressive lenses, tints, and UV coatings.
Vision Care. During the term of the Agreement, the Colleges agree to pay seventy-five per cent (75%) of the premiums for a Vision Care Plan providing coverage to a maximum of four hundred dollars ($400.00) each two (2) years for persons eighteen (18) years of age and over and four hundred dollars ($400.00) each one (1) year for persons under eighteen (18) years of age for glasses, frames, and contact lenses, subject to eligibility requirements and enrolment requirements, and the balance of the premium shall be deducted by payroll deduction.
Vision Care. The County shall provide vision care coverage for workers and their eligible dependents. The County will pay the entire premium for this coverage.

Vision Care. Effective February 1, 2003, the Board shall pay 90 per cent of the premium for Vision Care that shall include $250.00 per adult per 24 months and $200.00 per child per 12 months.
Vision Care. The Plan does not provide Benefits for vision care or eye examinations except as described in section 4.B. The Plan does not provide Benefits for vision therapy, including treatment such as vision training, orthoptics, eye training, or eye exercises. No Benefits are provided for deluxe or designer glasses or frames. No Benefits are provided for safety glasses and accompanying frames. Except as provided under section 4.B, the Plan does not provide Benefits for the prescription, fitting, or purchase of glasses or contact lenses.
