Your Coverage Under this Policy. You have purchased a health insurance Policy from Us. We will provide the benefits described in this Policy to You and Your covered Dependents. You should keep this Policy with Your other important papers so that it is available for Your future reference. Xxxxx is not a a member of any guaranty fund, including the Ohio Life and Health Insurance Guaranty Association. In the event of Xxxxx’s insolvency or in the event that Xxxxx ends operations, You are protected only to the extent that the hold harmless provision required by the Ohio Revised Code section 1751.13 applies to the health care services rendered. This hold harmless provision states that with the exception of an Annual Deductible, Copayment, Coinsurance and non Covered Services, Network Providers may not bill You for Covered Services. If You are receiving a course of treatment from Network Providers when Xxxxx ends operations or is declared insolvent, Covered Services will continue to be provided by Network Providers as needed to complete any Medically Necessary procedures and follow-up care for that course of treatment. If You are receiving Inpatient Services at a Network Hospital, your coverage for such Inpatient Services will be continued for up to thirty (30) calendar days after the Plan’s insolvency or end of operations. In the event of Xxxxx’s insolvency or end of operations, You may have to pay for health care services You receive from a Non-Network Provider, whether or not Oscar authorized the use of the Non-Network Provider.   • Workers’ Compensation and Employer’s Liability Coverage The insurer shall agree to waive all rights of subrogation against the City, its directors, officials, officers, employees, agents and volunteers for losses paid under the terms of the insurance policy which arise from work performed by the Consultant.

  • When Your Coverage Begins Your coverage will begin on the first day of the month following your eligibility date as long as we receive required enrollment information within the first thirty (30) days following your eligibility date and the premium is paid. If you or your dependents fail to enroll at this time, you cannot enroll in the plan unless you do so through an Open Enrollment Period or a Special Enrollment Period.

  • ’ Compensation and Employer’s Liability Coverage The Grantee shall provide workers’ compensation, in accordance with Chapter 440, F.S. and employer liability coverage with minimum limits of $100,000 per accident, $100,000 per person, and $500,000 policy aggregate.   • Worker's Compensation and Employer's Liability Insurance The Contractor shall have in effect during the entire life of this Agreement Workers' Compensation and Employer's Liability Insurance providing full statutory coverage.   • General Requirements for Insurance Coverage and Policies A. All required insurance policies shall be maintained with companies that may lawfully issue the required policy and have an A.M. Best rating of at least A- / “VII” or a Standard and Poor’s rating of at least A, unless prior written approval is obtained from the City Law Department.
