Cancellation of Housing. Contract: Students who wish to cancel or terminate this housing contract must follow the procedure set out in sections 1.28 (cancellation of Housing Contract before move-in date) or 1.29 (Termination of Housing Contract after move-in-date). I confirm that I have reviewed these sections. Move-out Date: unless agreed in writing by an authorized representative of UFV Housing Operations, means 24 hours after your last final exam listed on the UFV Registrar’s final exam schedule, or, in the case of students that do not have exams listed on the UFV Registrar’s final exam schedule, the move-out date will be the last day of classes listed by the UFV Registrar. Final exams listed by instructors on course outlines, as well as final projects/assignments are not considered final exams for the purpose of determining your move-out date. Contract extensions can be made in writing as per section 1.05 (Contract Extension) of The Handbook. Alcohol, Smoking and Cannabis: I confirm that I have reviewed section 3.02 of the Handbook regarding the use of controlled substances and section 3.03 regarding Smoking, incense, and flammables. Communication: Students are responsible for checking their mail and the email address listed on your StarRez Housing Portal profile regularly throughout your contract term. Upon signing this Contract or, in any event, before July 31, 2022, you must update your StarRez Housing Portal profile to include your UFV student email address and UFV student number.

  • Provider of health care means a physician or physician assistant licensed pursuant to chapter 630, 630A or 633 of NRS, perfusionist, dentist, licensed nurse, dispensing optician, optometrist, practitioner of respiratory care, registered physical therapist, podiatric physician, licensed psychologist, licensed audiologist, licensed speech-language pathologist, licensed hearing aid specialist, licensed marriage and family therapist, licensed clinical professional counselor, chiropractor, licensed dietitian or doctor of Oriental medicine in any form.

  • EEA Regulations means the Immigration (European Economic Area) Regulations 2006.

  • Administration of Medication means the act of placing a medication in or on an individual's body by a staff member who is responsible for the individual's care.

  • Out-of-home care means placement in a foster family home or
