Credit Monitoring and Insurance Services means the services to be provided to Settlement Class Members by Identity Guard, as further set forth in ¶ 4.6 of this Agreement.

Examples of Credit Monitoring and Insurance Services in a sentence

  • Class Members who have been identified from Premera’s records and who submit by the Claims Deadline a valid Claim Form approved by the Settlement Administrator may qualify to receive Credit Monitoring and Insurance Services, cash payments for Out-of-Pocket Losses or the Default Settlement Payment, and a California Payment.

  • Any such Class Member who does not submit a timely Claim Form in accordance with this Opinion and Order shall not be entitled to receive Credit Monitoring and Insurance Services, cash payments for Out-of-Pocket Losses or the Default Settlement Payment, and a CaliforniaPayment, but shall nevertheless be bound by any final judgment entered by the Court.

  • Submitting this Claim Form will not automatically enroll you into Credit Monitoring and Insurance Services.

  • The Credit Monitoring and Insurance Services (“CMIS”) option will include credit monitoring, fraud consultation, and identity theft restoration services.

  • You are eligible to make a claim for the Credit Monitoring and Insurance Services being offered through this Settlement even if you did not sign up for the previous services.

  • If you wish to receive Credit Monitoring and Insurance Services, you must check off the box for this section, provide your email address in the space provided in Section II, above, and return this Claim Form.

  • If you make a valid claim for Credit Monitoring and Insurance Services, the Settlement Administrator will send you information on how to activate your credit monitoring after the Settlement becomes final.

  • In the alternative to a Cash Fund Payment, you may elect Credit Monitoring and Insurance Services.

  • Credit Monitoring and Insurance Services provide a way to protect yourself from unauthorized use of your personal information.

  • If you file a Claim Form for a Documented Loss Payment and it is rejected by the Settlement Administrator and you do not correct it, and you have not otherwise claimed Credit Monitoring and Insurance Services and/or a Lost Time Payment, your Claim Form will be considered as an alternative claim for a Cash Fund Payment.

Credit Monitoring and Insurance Services means the credit monitoring and identity theft insurance solution to be offered as a Settlement Benefit under this Settlement. This Settlement Benefit is discussed in more detail in Section 3.4 below.

Related to Credit Monitoring and Insurance Services

  • Support and Maintenance Services means the support and maintenance services described in Section 1 of these Support Terms.

  • Maintenance Services means SAP’s then-current maintenance and/or support services offered under and described in detail in the applicable SAP PartnerEdge Model.

  • IT Services means any services relating to the IT Systems or to any other aspect of the Group’s data processing or other technology requirements, including software development, support or maintenance, consultancy, source code deposit, recovery and network services, facilities management or hardware maintenance; and

  • audit services means the professional services rendered by the Corporation’s external auditor for the audit and review of the Corporation’s financial statements or services that are normally provided by the external auditor in connection with statutory and regulatory filings or engagements;

  • Rehabilitation counseling services means services provided by qualified personnel in individual or group sessions that focus specifically on career development, employment preparation, achieving independence, and integration in the workplace and community of a student with a disability. The term also includes vocational rehabilitation services provided to a student with disabilities by vocational rehabilitation programs funded under the Rehabilitation Act of 1973, as amended.

  • Corporate Services Agreement means the Corporate Services Agreement dated as of the date hereof between the Borrower and the Services Provider, as amended from time to time in accordance with the terms hereof and thereof.

  • Outpatient services means those services rendered in a practitioner’s office or in the department of an approved facility where services are rendered to persons who have not had an overnight stay and are not charged for room and board.

  • Commercial Services means any waste management service, relating or connected to accumulating, collecting, managing, recycling, sorting, storing, treating, transporting, disposing, buying or selling of waste or any other manner of handling waste excluding services rendered by the municipality;

  • Financial Services Provider means any Lender and any other provider of financial services or products;

Respite may be provided for a defined period of time; respite is either planned or provided in response to a crisis.

  • Hospice services means palliative and supportive care and other services provided by an interdisciplinary team under the direction of an identifiable hospice administration to terminally ill hospice patients and their families to meet the physical, nutritional, emotional, social, spiritual, and special needs experienced during the final stages of illness, dying, and bereavement, as defined in Minnesota Statutes, § 144A.75, subd. 8, and includes the set of services as determined by the Medicare program under §1861(dd) of the Social Security Act and defined in 42 CFR § 418.3.

  • Replacement Services means any services which are substantially similar to any of the Services and which the Customer receives in substitution for any of the Services following the Call Off Expiry Date, whether those services are provided by the Customer internally and/or by any third party;
