The Requesting Employee (Recipient) Must: • Provide documentation for a non-work related seriously disabling illness or injury, as certified by a physician; • Have first exhausted all accrued sick leave, compensatory time, floating and vacation leave; • Have worked one full year at the City and have received satisfactory performance evaluations; and • Submit a request for donated leave to their Department Head or designee indicating reason and anticipated amount of lost work time.
If an Employee Recipient ceases employment with the Company for any reason other than death, disability, or retirement (as described above) or if a Recipient other than an Employee Recipient ceases affiliation with the Company for any reason other than death, an Option granted to the Recipient shall lapse immediately following the last day that the Recipient is employed by or affiliated with the Company.
Except as provided in Section 6(l), a Nonqualified Stock Option granted to an Employee Recipient shall expire on the tenth anniversary of its Date of Grant or at such earlier or later date as is set by the Committee in establishing the terms of the Nonqualified Stock Option at grant.
If the newly hired employee has received the New Employee Recipient Rights Orientation training from the Authority or a reciprocal entity within three years prior to the date of hire, Service Provider shall provide documentation to the Authority’s XXX.
During the one-year period following the Employee Recipient’s termination of employment by reason of disability, the Incentive Stock Option may be exercised as to the number of Shares for which it could have been exercised at the time the Employee Recipient became disabled, subject to any adjustments under Section 10 herein.
