Extended Medical Leave of Absence with Pay, the University will require a Certification of Health Care Provider form from the Faculty Member’s or Immediate Family Member’s medical provider, citing the effective date of the Leave and the final date of medical certification.
Normally, the decision about whether a Faculty Member should take or continue using sick leave or Extended Medical Leave or return to active faculty status will be made by the Faculty Member following consultation with his or her medical provider.
Should the Minister suffer an illness, injury, or disabling condition that continues after all accrued sick, vacation, and study leave has been exhausted, the Congregation will place the Minister on “Extended Medical Leave.” Extended Medical Leave will not extend beyond the earlier of the commencement of long-term disability benefit or 90 days following the exhaustion of all sick, vacation, and study leave.
In the event that the Minister is found eligible for long-term disability benefits and/or is absent from work or physically unable to perform duties at the conclusion of the Extended Medical Leave outlined in Section 4.7, the Congregation has the right to terminate the Minister’s employment upon written notice to the Minister.

Any absence (i.e., FML, Extended Medical Leave, Injury Leave, Military Leave) in excess of thirty (30) calendar days shall toll the above retention periods for the duration of the absence and extend the force and effect period for the applicable disciplinary action.
If additional leave is necessary, the employee’s physician shall submit the District’s Extended Medical Leave Form verifying that the employee is unable to return to work and additional leave, paid or unpaid, shall be granted accordingly.
