health services provider means health insurance carriers, pre-paid health plans, hospitals, physicians and other health care professionals, and other entities who contract with the Authority for the delivery of health care services to state and education employees and persons covered by the state Medicaid program; and
health services provider means a General Practitioner, Specialist or registered practising member of certain professions allied to medicine practising in private practice who Southern Cross approves for the provision of Healthcare Services under this Policy.
