Patient Safety Incident definition

Patient Safety Incident means any unintended or unexpected incident that occurs in respect of a Service User that could have led or did lead to, harm to that Service User;

  • The Provider will consider and respond to the recommendations arising from any audit, Serious Untoward Incident report or Patient Safety Incident report produced by the Commissioner.

Patient Safety Incident means, in relation to a health service provider,

Term: the period of one calendar year from the Commencement Date to the Expiry Date Termination Date: the date of expiry or termination of this agreement. Termination Payment Default: is defined in Schedule 3.

  • health and safety file means a file, or other record containing the information in writing required by these Regulations "health and safety plan" means a site, activity or project specific documented plan in accordance with the client's health and safety specification;

A critical incident may involve multiple individuals or results.

  • QAP shall have the meaning set forth in Clause 11.2;
