Relevant PHI definition

Relevant PHI means personal health information (as defined in the Personal Health Information Protection Act, 2004, S.O. 2004, c. 4) contained in OLIS, namely, laboratory results reported from and entered by Participating Laboratories, including each patient’s name, date of birth, age and sex, health number or other primary identifier, symptomatic and specimen information and test results; and
Relevant PHI means the Personal Health Information and sociodemographic data listed in Appendix “2” to this Agreement, entitled “Relevant PHI”;
Relevant PHI means the Personal Health Information that is collected, used, or disclosed through the Solution for the Purpose.

Examples of Relevant PHI in a sentence

  • Immediately notify the Ministry of any breaches of the Agreement, including this Schedule, any breaches of applicable law (including PHIPA) in connection with Relevant PHI accessed from or available through the Solution, or any other matter that could reasonably be regarded as a privacy or security breach in connection with the Solution.

  • The Recipient acknowledges that, subject to the terms and conditions of FIPPA, including any protection afforded to the Relevant PHI by FIPPA, the Recipient may be required to disclose information including copies of Reports in the custody or control of the Province, pursuant to FIPPA.

  • However, nothing in this Agreement shall be construed so as to limit the Province’s ability to use and disclose the Relevant PHI in accordance with Applicable Law.

  • If the Province provides a new Appendix in accordance with section 4.1, the new Appendix will be deemed to be either Appendix 1 Permitted Purpose, Appendix 2 Relevant PHI, Appendix 3 Roles and Responsibilities of the Province and the Recipient, Appendix 4 Security Requirements or Appendix 5 Consent Form as the case may be, for the period of time to which it relates.

  • The Ministry shall only use and disclose the Relevant PHI in accordance with this Agreement and Applicable Law.

  • The Relevant PHI is all of the following Personal Health Information and sociodemographic data of the patients who provided their consent using the Consent Form in Appendix 5 to the Recipient’s disclosure of this information to the Province and to the Province’s collection and use of this information for the Permitted Purpose.

  • This Agreement constitutes the entire agreement between the Parties with respect to the disclosure of the Relevant PHI in OLIS by the Laboratory to Ontario, and supersedes any prior agreements or understandings, collateral, oral or otherwise, existing between the Parties at the time this Agreement is entered into.

  • Only provide access to Relevant PHI in the Solution to the User’s End Users that require such access in order to perform their work on the User’s behalf and provide such End Users only with access to the minimum amount of Relevant PHI in the Solution that they need in order to perform their work for the User.

  • The Laboratory shall provide the Relevant PHI to Ontario at such time or times, and by such means, as may be agreed to by the Parties.

  • The Ministry, in its role as a HINP, has no intellectual property right, title or interest in the Relevant PHI.


More Definitions of Relevant PHI

Relevant PHI means the PHI listed in an Appendix, as it may be amended from time to time, which is relevant to generating a BI Report. 2.0 PROVISION, AUTHORISED ACCESS AND USE 2.1 No ownership in or to BI Reports is transferred to Participant by virtue of this Schedule and Participant has only the limited right to access and use BI Reports as set out in this Schedule. 2.2 Participant acknowledges that the BI Access Services are provided to Participant solely for the access to and use of BI Reports by Participant and its Users in connection with the Permitted Purposes, and not for any other use or use by any other person. Participant will ensure that it will not permit any other use or any other person to use the BI Access Services. 2.3 Upon request by KHSC, the Participant will provide a copy of any privacy or security assessments conducted on any Participant Systems that are involved in the access to or use of the BI Access Services, including BI Reports, as soon as reasonably possible. Any such assessments will be deemed to constitute Confidential Information of Participant and may only be disclosed to thirdparties or used by KHSC to assess the privacy and security of Participant’s systems in respect of the BI Access Services.2.4 Use of BI Reports. As a condition of Participant receiving BI Access Services and of any User and/or Participant gaining access to BI Reports, the Participant agrees to be responsible for:
Relevant PHI means the Personal Health Information listed in Schedule D hereto entitled “Relevant PHI”
Relevant PHI means the Personal Health Information listed in Appendix 2 hereto, titled “Relevant PHI”.

Related to Relevant PHI

  • relevant person means, in relation to premises in respect of which there is in force a premises licence -

  • Relevant Persons means all persons that undertake in Northern

  • relevant position , in relation to a business, means the position of director or manager, or other executive position, however that position is designated;

  • Relevant Party means the Borrower, each of the Owners, any other Security Party and each member of the Group from time to time;

  • Relevant Parties means the Agent, each Borrower, each Security Party, the Security Trustee, each Lender and the Swap Bank;

  • Relevant Period means the period from the passing of this resolution until whichever is the earliest of:

  • Relevant Ship means the Ships and any other vessel from time to time (whether before or after the date of this Agreement) owned, managed or crewed by, or chartered to, any Relevant Party;

  • relevant power means any power, whether exercisable by voting or otherwise and whether excisable alone or in association with others-

  • relevant week means the reduction week or part-week in which the capital in question of which the applicant has deprived himself within the meaning of paragraph 67(1)—

  • Relevant Page means the relevant page on Bloomberg or such other information service provider that displays the relevant information.

  • Relevant Price or "S(k)" means, in relation to a Fund Interest and any Strategy Calculation Date, the Fund Value per Fund Interest.

  • relevant asset means any part of the Network operated from time to time

  • Relevant Personal Data means Personal Data in respect of which we are the Controller.

  • relevant payment means either a student loan or an amount intended for the maintenance of dependants referred to in paragraph 76(7) or both.

  • Relevant Assets means all of the following:

  • relevant Minister means the Secretary of State or the Minister for the Cabinet Office;

  • Security Party means the Borrowers, the Corporate Guarantor, the Shareholder or any other person who may at any time be a party to any of the Security Documents (other than the Banks);

  • Relevant Group means the Company and any affiliated, combined, consolidated, unitary or similar group of which the Company is or was a member.

  • Relevant Amount means the amount of contributions which would have been required to be made by the relevant employer had such contributions been obligatory under the Central Provident Fund Act in respect of —

  • relevant provision means a provision in any subordinate legislation made under the Act or made, in connection with the Act or such provision, under any other enactment;

  • Relevant Property means property of a kind affected by the transfer order, whether it is an estate or interest in land or any other property.

  • relevant service means service that counts towards the 30 days’ qualifying service for the purposes of subsection (1).]

  • the relevant period means the period beginning at the commencement of the qualifying period and ending on the date when all the requirements of paragraph (A)(i) to (iv) above have been satisfied.

  • Relevant Percentage means, in respect of any Subsidiary Issuer, the percentage as specified in the relevant Pricing Supplement;

  • the Group means the Company and its subsidiary undertakings (if any); and

  • relevant scheme means any other scheme approved or seeking approval under Chapter 1 Part XIV of the Act and in respect of a Class A Member who is a Controlling Director also any retirement annuity contract or trust scheme approved under Chapter III Part XIV or any personal pension scheme as approved under Chapter IV Part XIV of the Act insofar as it provides benefits secured by contributions in respect of Service.
