Transition Management means the services and supports offered to an Individual to assist them transition from a residential setting to an independent living setting of their choice.

Transition Management means the process of assessing and reassessing patients for post-hospital needs and then developing and implementing a plan to coordinate those services identified as necessary for the patient when they leave the hospital.
