Common use of Absence Reporting Clause in Contracts

Absence Reporting. 20.01 Proof of illness shall be established in every case by a qualified practitioner where illness is of more than five (5) days duration. On return to duty following an illness of more than five (5) days, the employee must present to his supervisor a doctor’s statement of verification that the employee is fit to return to duty. The Employer may request a physician’s medical certificate to verify absences from work. If requested by the Employer, the employee will be reimbursed for the cost of the medical certificate to a maximum of twenty-five dollars ($25.00). It is understood the employee will provide proof of the cost of the certificate.
