ACCIDENTAL DEATH AND DISMEMBERMENT BENEFIT. The Employer provides a life insurance amount of two (2) times the full-time employee’s basic annual earnings, rounded up to the next multiple of one thousand dollars ($1,000). For part-time employees, the amount payable is twelve thousand and five hundred dollars ($12,500).
ACCIDENTAL DEATH AND DISMEMBERMENT BENEFIT. The Employer agrees to pay the full cost of premiums for the Accidental Death and Dismemberment benefit for eligible employees. In addition to the group life insurance benefit provided under Section 3.01, a Group Accidental Death and Dismember‑ ment benefit will be provided while eligible as an active em‑ ployee: Effective June 1, 2006 – $36,000.00 Effective June 1, 2007 – $37,000.00 Effective June 1, 2008 – $38,000.00 The above principal sum will be payable in the case of death or loss of both hands, both feet, or sight of both eyes, or any two of the above members, providing death or such loss oc‑ curs within 90 days of the date of the accident and is caused by external, violent and accidental means. One‑half of the above principal sum will be payable in the case of loss of one hand, one foot, or sight of one eye under the same conditions. The policy under which this benefit is provided will contain such limitations and conditions as are found in a standard Group Accidental Death and Dismemberment policy issued in the province of Ontario. The maximum payable under this benefit for any one accident is the principal sum.

ACCIDENTAL DEATH AND DISMEMBERMENT BENEFIT. 3.01 Accidental Death and Dismemberment Insurance is payable if accidental bodily injury is sustained by an employee while the employee is insured for this Benefit Provision and such injury results directly and independently of all other causes in any one of the losses set forth in Article 3.02. Benefits will be paid for certain losses, provided that:
(i) for losses set forth in Section A, the loss is incurred within 365 days of the date of accidental bodily injury, and
(ii) for losses set forth in Section B, the loss is incurred within 365 days of the date of accidental bodily injury and is continuous for a period of one year.

3.03 Accidental Death and Dismemberment Benefits are not payable if the death or injury of the employee results from or was in any manner or degree associated with or occasioned by:
(i) suicide or wilfully self-inflicted injuries, while sane or insane;
(ii) viral infections; bacterial infections (except pyogenic bacterial infections which occur with and through an accidental bodily injury). any form of disease or illness or physical or mental infirmity; medical or surgical treatment;
(iii) service, including part-time or temporary service in the armed forces of any country.,
(iv) war or hostilities of any kind, whether or not the insured person was actually participating therein, and whether or not war was declared;
(v) committing or attempting to commit a criminal offence as defined in the Criminal Code of Canada;
(vi) air travel, ascent or descent, where:
(a) an insured employee is acting in any capacity as a member of the crew,
(b) any loss occurs as a result of travel in an aircraft owned, leased or rented by the Employer.
