Accidental Death and Dismemberment Benefits. (i) All 24 hour company paid accidental death and dismemberment insurance benefits, will cease at the commencement of the Termination Leave.
(ii) All Executive, spousal and child optional accidental death and dismemberment benefits will cease at the commencement of the Termination Leave; the Executive may elect to convert all or part of his Executive and/or spousal coverages to individual coverage, within 31 days after the commencement of the Termination Leave.
Accidental Death and Dismemberment Benefits. (1) accident shall be in an amount that is equal to one (1x) times the employee's annual earnings rounded to the next higher one thousand ($1,000) dollars to an overall maximum of twenty-five thousand ($25,000) dollars. Accidental death and dismemberment benefits shall be paid to the employee's named beneficiary in the event of death due to any cause and if no beneficiary is named then benefits shall be paid to the employee's estate. Accidental death and dismemberment benefits are reduced by fifty (50%) percent at age sixty-five (65) and terminate at age seventy (70), retirement, or termination of employment.
Accidental Death and Dismemberment Benefits. All employees shall be entitled to accidental death and dismemberment benefits for themselves. The principle sum that is payable for injuries resulting from any one (1) accident shall be a maximum of twenty-five thousand ($25,000.00) dollars.
Accidental Death and Dismemberment Benefits. 3.01 Accidental Death and Dismemberment Insurance is payable in the event of death which results from injury caused accidentally and within 365 days of such injury on the basis of 100% of the amount set out in paragraph 3.02. Benefits will be paid for certain dismemberment's as set out in paragraph 3.02.
3.02 The amount of Accidental Death and Dismemberment Insurance shall be: - Loss of Life $30,000.00 Loss of Both Hands Loss of Both Feet Loss of the Sight of Both Eyes. Loss of One Hand and One Foot Loss of One Hand and the Sight of One Eye Loss of One Foot and the Sight of One Eye Loss of One Hand $15,000.00 Loss of One Foot Loss of the Sight of One Eye
3.03 Accidental Death and Dismemberment Benefits are not payable for any loss resulting directly or indirectly, wholly or partially from any of the following causes: -
(a) Suicide or willfully self-inflicted injuries, while sane or insane;
(b) Committing or attempting to commit a criminal offense as defined in the Criminal Code of Canada;
(c) War or hostilities of any kind, whether or not the insured person was actually participating therein, and whether or not war was declared;
(d) Injuries sustained by the insured person as the result of driving a vehicle when the injuries were sustained, the blood of the insured person contained in excess of eighty (80) milligrams of alcohol per one hundred (100) milliliters of blood.

Accidental Death and Dismemberment Benefits. A. We Agree to Pay Benefits for Loss From Bodily Injuries:
1. Caused by an accident which happens while a Member is covered by this policy; and
2. Which, directly and from no other causes, result in a covered loss as described in the following Section.
Accidental Death and Dismemberment Benefits. The following benefit is being underwritten by Blue Cross Life Insurance Company of Canada.
Accidental Death and Dismemberment Benefits double amount of the employee's Life Insurance (one hundred thousand ($100,000.00) dollars).
