Admission Agreement. D13.1 You must ensure that each Resident, or their nominated representative, signs an Admission Agreement, either on the day that the Resident commences receiving Services at your Facility, or, in the case of an Emergency Admission, as soon as is reasonably practicable but no later than 10 Working Days after the Resident is admitted. You must not charge the Resident or any other Person for preparing or providing an Admission Agreement. D13.2 A copy of any or all Admission Agreements between you and your Residents must be made available to us on request and without charge. D13.3 The Admission Agreement must contain: a. advice that explains that the Resident has the right under the Residential Care and Disability Support Services Act to apply for a review of the Resident’s Means Assessment; b. a list of items that are excluded from the Services as set out in clause D14;

Admission Agreement. Should the resident be required to temporarily leave the facility for a therapeutic leave or hospitalization, the terms and conditions of the resident’s original admission agreement shall remain in effect during the course of any bed hold period (or lack thereof) and remain in effect until the resident’s readmission to the facility. Admission agreements do not remain in effect once a resident is officially discharged from the facility. Hospital or therapeutic leaves that result in official discharge may require the execution of a new admission agreement.

The Admission Agreement is a legally binding contract that defines the rights and obligations of each person (or party) signing the contract. The admission services manager or other facility staff member will take time to review the Admission Agreement with you. You or your family are responsible for providing clothing. We encourage residents to dress in regular clothing if possible, and we recommend that you bring several changes of clothing. Be sure to include active wear such as sweats and tennis shoes that may be worn during therapy. We strive to protect and preserve the personal property of each of our residents and recommend items are marked with your name for easy identification. Clothing items requiring dry cleaning are the responsibility of the resident. At the time of your admission we will give you the option to have laundry cleaned by your family, the facility, or an outside laundry service. Please keep in mind the facility uses industrial detergents in commercial machines on very high heat. We recommend not bringing clothing items that are delicate or cannot tolerate high heat. All clothing and other personal items should be listed on the inventory check list when you are admitted. Please notify the charge nurse of any additional items brought into the facility and any items taken out of the facility.

You must not charge the Subsidised Resident or any other Person for preparing or providing an Admission Agreement. D13.2 A copy of any or all Admission Agreements between you and your Subsidised Residents must be made available to us on request and without charge. D13.3 The Admission Agreement must contain: a. Advice that explains that the Subsidised Resident has the right under the Social Security Act to apply for a review of the Subsidised Resident’s means assessment; b. A list of items that are excluded from the Services as set out in clause D14; c. An itemised list of each additional service offered by you which is not part of the Services under this Agreement and the charge for each additional service; d. A statement of the right of the Subsidised Resident to: i. be admitted to and reside in your Facility without being required to receive and pay for any additional service; ii. decide whether to receive any individual additional service; iii. at any time decide to receive or cease to receive any individual additional service; e. The extent of your liability for damage or loss of the Subsidised Resident’s personal belongings, including clothing; f. Provisions relating to the following topics: i. staffing of the Facility; ii. safety and personal security of Residents; iii. fire protection and emergency management; iv. communication with Residents for whom English is a second language or whose ability to communicate is limited; g. A provision which informs the Subsidised Resident that the contact details of the Subsidised Resident’s family members or nominated representatives provided to you by the Subsidised Resident under clause D13.4, may be provided to us for the purposes of this Agreement, including, without limitation: i. during the course of or following an Audit; or ii. if this Agreement is varied or terminated under clauses A16, A23 or A24; or iii. when we take action under clause A22, and otherwise as permitted by the Privacy Xxx 0000 and any code of practice issued under that Act covering Health Information held by providers; h. The procedure that a Subsidised Resident must follow if he or she wishes to make a complaint about you or any of the services received by the Subsidised Resident; i. A description of transportation policies, procedures...
