Xxxxxx Youth. Education Project FYEP The Xxxxxx Youth Education Project is co-facilitated by the Xxxxxx County Office of Education Xxxxxx Youth Services Coordinator and the Xxxxxx County Presiding Juvenile Court Judge and acts as the advisory group for the Xxxxxx County Xxxxxx Youth Services Program. The membership is comprised of representatives of agencies that have a role in educating xxxxxx youth. The committee meets monthly to provide oversight to the coordination and educational development of all children within the delinquency/dependency system. Xxxxxx Youth Services FYS Grant-funded program for each county office of education and selected school districts to increase interagency support for xxxxxx youth. The Xxxxxx County FYS program is aimed towards improving the educational outcomes of wards and dependents, ages 4 to 21 who are enrolled in the school system.
Xxxxxx Youth. Is defined as a child who has been removed from the custody of his or her parent(s) or guardian(s) by the juvenile court and placed in xxxxxx care. (See Education Code Section 48853.5(a) and 45 CFR 1356.21(k)). Xxxxxx care means 24-hour substitute care for children placed away from their parents or guardians and for whom the child welfare agency has placement and care responsibility. This includes, but is not limited to, placements in xxxxxx family homes, xxxxxx homes of relatives, group homes, emergency shelters, residential facilities, child care institutions, and pre-adoptive homes. (45 CFR 1355.20). A student in xxxxxx care, under the Local Control Funding Formula (LCFF) definition, who lives at home with both or either biological parent, are not entitled to ESSA mandates and provisions. Assembly Bill (AB) 490 Educational Liaison - Every school district must have an appointed educational liaison to serve Xxxxxx Youth Education Code 48853.5(c). AB 490 Educational Liaison for each district will also serve as the district point of contact (POC).
Xxxxxx Youth. An individual between the ages of birth (0) to eighteen (18) years, 11 referred for xxxxxx care services. 12 Health and Education Passport (HEP): The document that provides historical and 13 current medical, dental, mental health, and educational information, as it pertains to a Xxxxxx Youth 14 or NMD. 15 HEP Encounter Form: The form to record the Xxxxxx Youth’s/NMD’s 16 medical/dental exam information for the Health Passport Update report. 17 Important Persons: Individuals identified by the Xxxxxx Youth/NMD placed in 18 STRTPs, age ten (10) years or older, as defined in WIC Section 366.3(e)(2), that are important to 19 the Xxxxxx Youth/NMD consistent with his/her best interest. 20 Individual Education Plan (IEP): An assessment procedure requested by parents, 21 guardians, school staff, and/or other involved parties, to determine a youth’s educational needs.
