AFSCME Dental Plan. Upon receipt of a signed authorization from an employee on a standard form acceptable to the Employer, the Employer agrees to deduct from such employee's pay the uniform individual or family plan premium for the AFSCME sponsored dental insurance plan. The Union will notify the Employer annually in writing of the amount of the uniform individual or family plan premium to be deducted. Deduction shall be made on each payday and shall be remitted, together with an itemized statement, to the Treasurer of the Union or his/her authorized designee by the 15th day of the month following the month in which the deduction is made.

The Union will notify the City annually in writing of the amount of the uniform individual or family plan premium to be deducted.   • Dental Plan (a) The Employer shall pay the monthly premium for employees entitled to coverage under a mutually acceptable plan which provides:
