After Hours Care, Emergency and Urgent Care. After Hours Non-Emergency Care (Weekdays 5:00 PM to 8:30 AM, Weekends and Holidays) There may be times when you need to receive advice or treatment for an injury or onset of an illness, which simply can't wait until regular ESP of HHSI Center hours. When you need non-emergency care after hours, there will always be a clinician available 24 hours a day, 7 days a week, and 365 days a year. When you call our after hours phone number, your call will be answered by an answering service who xxxx xxxx the on-call ESP clinician to assist you. For after hours non-emergency care, call 000- 000-0000 ESP of HHSI covers emergency care for an emergency medical condition. In an emergency, please call 911. An emergency medical condition is one that manifests itself by acute symptoms of sufficient severity (including severe pain) such that a prudent lay person, who possesses an average knowledge of health and medicine, could reasonably expect the absence of immediate medical attention to result in: (1) placing the health of the individual in serious jeopardy; (2) serious impairment to bodily function; or (3) serious dysfunction of any bodily organ or part. Prior authorization is not needed for emergency care. If you call 911, it is important that you have someone notify ESP of HHSI as soon as possible and let us know what has happened. Your physician at ESP of HHSI will be called immediately to coordinate your care. If you are hospitalized in a facility other than a contracted hospital we will arrange for you to be transferred to a contracted hospital once your medical condition has stabilized. This will ensure that your care can be coordinated by your ESP of HHSI Interdisciplinary Team. If you receive emergency care when you are temporarily outside of the ESP of HHSI area, you should ask the provider to bill ESP of HHSI. Bills for these services can be sent to: Elder Service Plan of the Harbor Health Services 0000 Xxxxxx Xxxxxx Mattapan, MA 02126 Attention: Financial Operations If you have paid for the emergency medical services you received outside of the ESP of HHSI area, you should request a receipt from the facility or physician involved. This receipt must show the provider’s name, date and type of treatment, and date of discharge, if hospitalized and the amount you were required to pay.
