Common use of Authorization for Usage Clause in Contracts

Authorization for Usage. ‌ Employees are entitled to use sick leave only when incapacitated due to sickness, injury or when receiving necessary medical or dental service, or in the event of an illness in the immediate family. Accrued sick leave may be used each fiscal year for family medical/dental preventive care appointments. The use of employee sick leave for family illness is limited to three (3) days per occurrence. Under circumstances causing an unusual hardship, and at the discretion of the department head, sick leave may be extended beyond the three day maximum. Evidence in the form of a physician’s certificate or certificate of illness executed by the employee may be required by the Department Head.
