Provide a Benefits Data Warehouse for the County to query a number of reports to be designed at implementation and throughout the term of the Contract, and allow County to use the Benefits Data Warehouse to generate Ad hoc reports. Data elements to be defined by the County during implementation.

Benefits Data Warehouse. Provide a Benefits Data Warehouse for the County to query a number of reports to be designed at implementation and allow County to use the Benefits Data Warehouse to generate Ad hoc reports. Provide the County with monthly active census reports showing all enrolled health plan participants and which reconciles to premium reports. In addition to demographic data, census reports must also include billing source. Census reports must also account for all participants on Direct Bill. Provide the County with monthly retiree census reports showing all enrolled health plan participants and reconciles to premium reports. In addition to the retiree demographic data, census reports must also include but not limited to following information: billing source ( i.e. OCERS pension), Retiree Xxxxx Xxxxxx, Xxxxx Xxxxx, Medicare Part B reimbursement. Provide the County with monthly active census reports for other benefits as requested. Reports to generate on a participant, agency and summary level for subscribers and dependents. System needs to be flexible enough to add reports or request reports based upon user defined criteria System needs to be able to retain historical data. Contractor to import the data so that it can be readily used by County Employee Benefits staff.

  • Diagnostic Services Procedures ordered by a recognized Provider because of specific symptoms to diagnose a specific condition or disease. Some examples include, but are not limited to:

  • Specific Services Contractor agrees to furnish the following services: Contractor shall provide the services described in Exhibit “A”. No additional services shall be performed by Contractor unless approved in advance in writing by the County stating the dollar value of the services, the method of payment, and any adjustment in contract time or other contract terms. All such services are to be coordinated with County and the results of the work shall be monitored by the Health and Human Services Agency Director or his or her designee.

Oracle does not monitor, and does not address issues with, non-Oracle software provided by You or any of Your Users that is stored in, or run on or through, the Services. Information collected by Oracle monitoring tools (excluding Your Content) may also be used to assist in managing Oracle’s product and service portfolio, to help Oracle address deficiencies in its product and service offerings, and for license management purposes.

  • Personnel Equipment and Material Engineer shall furnish and maintain, at its own expense, quarters for the performance of all Engineering Services, and adequate and sufficient personnel and equipment to perform the Engineering Services as required. All employees of Engineer shall have such knowledge and experience as will enable them to perform the duties assigned to them. Any employee of Engineer who, in the reasonable opinion of County, is incompetent or whose conduct becomes detrimental to the Engineering Services shall immediately be removed from association with the Project when so instructed by County. Engineer certifies that it presently has adequate qualified personnel in its employment for performance of the Engineering Services required under this Contract, or will obtain such personnel from sources other than County. Engineer may not change the Project Manager without prior written consent of County.

  • School Improvement Plan As permitted under IC § 20-10.2-3-1.5, the Charter shall serve as the Charter School's strategic and continuous school improvement and achievement plan (hereafter, the "School Improvement Plan"). To the extent that IC § 20-10.2 applies to the Charter in its function as the School Improvement Plan, the Organizer shall comply with the requirements under IC § 20-10.2.

  • School Improvement Plans The School shall develop and implement a School Improvement Plan as required by section 1002.33(9)(n), Florida Statutes and applicable State Board of Education Rules or applicable federal law.

  • Chiropractic Services This plan covers chiropractic visits up to the benefit limit shown in the Summary of Medical Benefits. The benefit limit applies to any visit for the purposes of chiropractic treatment or diagnosis.

  • EMPLOYEE EXPENSES AND MATERIALS 9.1 The District shall pay the full cost of the purchase, lease, or rental of uniforms, equipment, identification badges, emblems, aprons, hairnets, and cards required by the District to be worn or used by employees.
