Change of Accommodations. The home reserves the authority to determine and make all arrangements regarding residency including; admission and dismissal of you and other residents and adjustments in rates and accommodations consistent with state law and home policies. Dual Occupancy: The home permits dual occupancy of selected rooms. If there are two of you, in the event of the death or transfer of one of you during the term of this agreement, the remaining resident may remain in your room upon payment of the current basic rate for occupancy, unless otherwise documented between you and the home administrator.

Change of Accommodations. The Community reserves the right to determine and make all arrangements regarding residency, including moving-in and moving- out of Residents and adjustments in rates and accommodations consistent with state law and our policies. The Monthly Rent may be adjusted from time-to-time to reflect the current rates of The Community for the plan and apartment size chosen.
Change of Accommodations. The resident and/or the responsible party hereby authorize Daughters of Xxxxx to change the room assignment of the resident at the discretion of the facility as a result of staff reassessment of the resident’s medical or psychosocial needs. The resident and/or the responsible party will be counseled by the social services staff before the resident’s room is changed. The resident, family or responsible party will be encouraged to participate in planning the move and will receive advance notice of the transfer.

  • Special Accommodations Any person requiring a special accommodation due to a disability should contact the Department’s Americans with Disabilities Act (ADA) Coordinator, at (000) 000-0000.
