Clinician Definitions. 16 a. Case Manager - A clinician who serves as the primary clinician for 17 designated patients that constitute their caseload; the Case Manager 18 both provides direct and ongoing care to their patients and coordinates 19 the patient’s plan of care. 20 21 b. Float - A clinician whose assignment varies according to the Clinical 22 Unit’s coverage needs who may carry a partial or temporary caseload. 23 24 c. Nurse - Registered Nurse currently licensed to practice professional 25 nursing in Oregon and/or Washington, including but not limited to: 26 i. Wound Ostomy Nurse - A Nurse holding a recognized board 27 certification to provide wound ostomy or continence care, or some 28 combination thereof, who also serves as a consulting resource. 29 30 ii. Psychiatric Mental Health Nurse - A Nurse whose duties include the 31 provision of mental health care to Home Health patients, who also 32 serves as a consulting resource. 1 iii. Palliative Care Nurse - A Nurse whose duties include the provision of 2 palliative care to Home Health patients, who also serves as a 3 consulting resource. 5 d. Physical Therapist - Licensed Physical Therapist employed to provide 6 skilled physical therapy services for the care of Home Health and 7 Hospice patients. These services include evaluation, treatment, and 8 consultation. 10 e. Occupational Therapist - Licensed Occupational Therapist employed to 11 provide skilled occupational therapy services for the care of Home Health 12 and Hospice patients. These services include evaluation, treatment, and 13 consultation. 15 f. Speech Language Pathologist - Licensed Speech Language Pathologist 16 employed to provide skilled speech, language, swallowing, voice, and 17 cognitive services for the care of Home Health and Hospice patients. 18 These services include evaluation, diagnosis, treatment, and 19 consultation. 20 i. Alternative and Augmentative Communication Speech 21 Language Pathologist (AAC-SLP) - Licensed Speech Language 22 Pathologist employed to provide specialty communication 23 services to those Home Health and Hospice patients who 24 require alternative and augmentative communication. AAC- 25 SLPs do not require a specialty certification and may become 26 an AAC-SLP through experience. Their primary caseload is 27 patients with alternative and augmentative communication 28 needs. 30 g.
