3.02 b) Within Fifteen (15) days after of an employee commences commencing employment, they shall have an amount equivalent to the monthly dues of the Union deducted from their wages, and that amount, along with the employee’s name, will be added to the current Union check-off, before same which is mailed to be emailed to the Union. The Employer will deduct commence deductions of the Initiation Fee from the employee starting in the first calendar month of employment and prior to following the calendar month in which the employee completing sixty (60) calendar days of employmentwas employed. The deduction of the Initiation Fee shall be in increments of fifty dollars ($50.00) per month commencing the first month following ninety (90) calendar days of employment until the Local Union Initiation is fully paid.
c) All employees referred to above, will be required to sign an authorization form for the deduction and remittance of Initiation Fees, Union Dues, and fines and/or assessments, which may be levied by the Union in accordance with the Union’s Constitution and/or By-Laws.
d) On the first pay period of each month, the Employer shall deduct and pay over to the Secretary- Treasurer of the Local Union, such Initiation Fees, Union Dues, fines and/or assessments levied in accordance with the Union’s By-Laws, owing by the said employees xxxxxxxxx to the said Union. Should the employee have no earnings during the first pay period of the month, the Employer will deduct as indicated on the next available pay period to reconcile the Local Union Checkoff. Monies deducted during any month shall be forwarded by the Employer to the Secretary-Treasurer of the Local Union, not later than the twentieth (20th) day of the month following the month to which such monies apply by Electronic Funds Transfer (EFT). The EFT shall be accompanied by an emailed statement which will include the names of the employees for whom the deductions were made, and the amount of each deduction. The Employer is entitled to rely absolutely upon a certificate of the Secretary-Treasurer of the Local Union that such fees, dues, fines and/or assessments were levied in accordance with the Local Union’s By-Laws. The Company will remit the amount indicated as per the Local Union Checkoff plus all dues collected for newly hired employees as per Article No. 4 b).
3.03 e) The Union shall forward all authorization forms to the Employer. It shall be the responsibility of the Employer to take proper and due care of all authorization forms sent to the Employer by the Union.
f) At the beginning of each year, the Employer shall provide a schedule to all employees detailing the pay periods in which Union dues shall be deducted.

3.2 The Employer shall remit each month to the Union, the amount of deductions made for that particular month including initiation fees, reinstatement fees, membership dues, and arrears, together with a list of employees with their social security numbers, hourly rate of pay, and arrearages per week/month, for whom such deductions have been made. The list will indicate all official personnel actions that result in a change in status of bargaining unit members, including new hires, terminations, leaves of absence, and layoffs. The remittance shall be forwarded not later than the 25th of the month following the month in which deductions are made. The Parties agree that they shall continue to meet and confer regarding the implementation of methods and processes that will improve the efficiency of compiling and transmitting information relevant to such deductions, including doing so electronically if possible.
3.3 The Employer's obligation is limited solely to making the authorized deduction and such obligation shall cease at the time the employee is terminated or laid off for lack of work, including summer layoffs.
3.4 The Union shall hold harmless the Employer from any and all claims that may arise out of the Employer’s compliance with this Article.
3.5 The Employer shall deduct monthly or weekly a flat dollar amount, if the Employer’s payroll system permits, from the gross wages or salary of each employee who voluntarily executes the Committee on Political Education (COPE) payroll deduction authorization form, the contributions so authorized on that form, and remit those contributions to the Union at the same time that the Employer remits to the Union the Union dues that are separately voluntarily authorized by employees to be deducted from their gross wages or salaries and remitted to the Union pursuant to Article 3, Section 3.2 of this Agreement. The Employer may remit COPE contributions and Union dues to the Union by a single check, or by separate checks. With each COPE contribution remittance, the Employer shall provide the Union with a written itemization setting forth as to each contributing employee his or her name, Social Security number and total contribution amount. The parties acknowledge that the Employer’s costs of administration of this COPE payroll deduction have been taken into account by the parties in their negotiation of this Agreement and have been incorporated in the wage, salary, and benefits provisions of this Agreement. The Employer's responsibility under this Section is limited solely to disbursing the funds to the Union as provided in this Section. The Union shall assume all responsibility for distribution of the COPE contribution remittance to the COPE's specified on the form.

DEDUCTION OF UNION DUES. 3.01 Section 1. The Employer agrees that upon receipt of an individual written request on a payroll deduction authorization form (which is attached to and made a part of the Agreement as Appendix
A) agreed to by the parties and signed by an employee, the Employer will deduct Union dues and an initiation fee from such employee's wages.
Section 2. The Employer will deduct and pay over forward the amount so deducted to the Secretary-Treasurer of the Union or his authorized agent.
Section 3. Payroll deductions for union dues will be made bi-monthly for properly executed deduction authorization forms received at the Employer Accounting Department on or before the fifth day of the preceding month.
Section 4. Deductions shall be remitted to the Secretary-Treasurer of the Union not later than twenty (20) days after the end of the preceding month during which deductions were made.
Section 5. The Employer agrees to furnish the Secretary-Treasurer of the Union, at the time of remitting the dues deducted, a list of employees' names, social security number and rate of pay for whom dues were deducted. Also, the names, addresses, and social security numbers of any monthly Union duesnew hires; delete the names of employees who resigned from the company, Initiation fees and/or assessments which may and include the names, addresses, and social security numbers of employees for whom no dues were deducted, including the reason for same.
Section 6. An employee's authorization will be levied automatically cancelled upon termination of employment. An employee's authorization shall be suspended upon leave of absence in excess of thirty (30) calendar days.
Section 7. Upon return from a leave of absence, the returning employee's deduction authorization shall be reinstated in accordance with the Union’s By- laws, owing by said Employees hereunder provisions of this Article.
Section 8. Any change in the amount of monthly union dues will be certified to the Union. The Employer will deduct the monies from the first pay of an Employee each month, and remit such monies to the Secretary-Treasurer of the Union on or before the twentieth (20th) day of the following month in which the monies are deducted, together with one (1) copy of the Check-off list as above mentioned. DEDUCTION OF UNION DUES. 3.01 Section 1: The Employer agrees to deduct from the salaries of Nurses their dues, when authorized in writing to do so by each Nurse who is a member thereof, so long as employed by the Employer.
Section 2: The Employer shall not be required to make any deductions from payroll for dues under Section 1 until JNESO shall have submitted a copy of the dues authorization card (see appendix A). JNESO will notify the Employer of the date to commence dues deductions.
Section 3: The Employer will only deduct and pay over to monthly the Secretary-Treasurer amount of dues shown on the written authorization signed by the Nurse during the time that such written authorization remains in effect. Such written authorization shall no longer be in effect when: a) The Employer shall have received a written revocation by the Nurse; or b) employment by the Nurse shall have been terminated by discharge, layoff, resignation or otherwise; or c) when the Nurse commences a leave of absence without pay; or d) the Nurse shall have resigned or is no longer a member of the Union, Association. It is mailed hereby agreed that JNESO waives any claims for unpaid dues if JNESO fails to notify the Union. The Employer will deduct the Initiation Fee from the employee starting the first month of employment and any discrepancies prior to three (3) succeeding month’s deductions.
Section 7: In the employee completing sixty (60) calendar days event of employment. Section 2. The Employer shall remit each month to the Union, the amount of deductions made for that particular month including initiation fees, reinstatement fees, membership dues, and arrears, together with a list of employees with their social security numbers, hourly rate of pay, and arrearages per week/month, for whom such deductions have been made. The list will indicate all official personnel actions that result in a change in status of bargaining unit members, including new hires, terminations, leaves of absence, and layoffs. The remittance shall be forwarded not later than the twenty-fifth (25th) of the month following the month in which deductions are made. The Parties agree that they shall continue to meet and confer regarding the implementation of methods and processes that will improve the efficiency of compiling and transmitting information relevant to such deductions, including doing so electronically if possible.
Section 3. The Employer's obligation is limited solely to making the authorized deduction and such obligation shall cease at the time the employee is terminated or laid off for lack of work, including summer layoffs.
Section 4. The Union shall hold harmless the Employer from any and all claims that may arise out of the Employer’s compliance with this Article.
Section 5. The Employer shall deduct monthly or weekly a flat dollar amount, if the Employer’s payroll system permits, from the gross wages or salary of each employee who voluntarily executes the Committee on Political Education (COPE) payroll deduction authorization form that is Appendix B to this Agreement the contributions so authorized on that form, and remit those contributions to the Union at the same time that the Employer remits to the Union the Union dues that are separately voluntarily authorized by employees to be deducted from their gross wages or salaries and remitted to the Union pursuant to Article 7, Section 2 of this Agreement. The Employer may remit COPE contributions and Union dues to the Union by a single check, or by separate checks. With each COPE contribution remittance, the Employer shall provide the Union with a written itemization setting forth as to each contributing employee his or her name, Social Security number and total contribution amount. The parties acknowledge that the Employer’s costs of administration of this COPE payroll deduction have been taken into account by the parties in their negotiation of this Agreement and have been incorporated in the wage, salary, and benefits provisions of this Agreement. The Employer's responsibility under this Section is limited solely to disbursing the funds to the Union as provided in this Section. The Union shall assume all responsibility for distribution of the COPE contribution remittance to the COPE's specified on the form that is Appendix B.

The Employer will, at the time of making each remittance hereunder to the Secretary-Treasurer of the Union, update the Union’s Pre-Billing statement showing all monthly dues submitted for Members along with current address, postal code, and date of hire. The Monthly Check-off List will reference any; • New Members to be listed in alphabetical order with current address, postal code, date of hire. • Terminations or resignations are to be clearly identified with current address, postal code and date of termination or resignation; • Any current address change to be updated as well as name changes (i.e. marriage). • If an Employee works anytime during a month, the Employer assures the Union that the total amount of the monthly dues as specified information supplied by the Secretary-Treasurer of the Union will be deducted and forwarded to the Local Union. 3.02 b) Fifteen (15) days after an employee commences employment, they shall have an amount equivalent to the monthly dues of the Union deducted from their wages, and that amount, along with the employee’s 's name, will be added to the Union check-off, before same is mailed to the Union. The Employer will deduct the Initiation Fee from the employee starting the first month of employment and prior to after the employee completing sixty has completed ninety (6090) calendar days of employment. The deduction of the Initiation Fee shall be in increments of fifty dollars ($50.00)) per month commencing the first month following ninety (90) calendar days of employment until the Local Union Initiation is fully paid.
3.03 c) All employees referred to above, will be required to sign an authorization form for the deduction and remittance of Initiation Fees, Union Dues, and fines and/or assessments, which may be levied by the Union in accordance with the Union's Constitution and/or By-Laws.
d) The Employer shall deduct and pay over to the Secretary-Treasurer of the Local Union, such Initiation Fees, Union Dues, fines and/or assessments levied in accordance with the Union's By laws, owing by the said employees hereunder to the said Union. Monies deducted during any month shall be forwarded by the Employer to the Secretary-Treasurer of the Local Union, not later than the twentieth (201h) day of the month following the month to which such monies apply, and shall be accompanied by a written statement which will include the names of the employees for whom the deductions were made, and the amount of each deduction. The Employer is entitled to rely absolutely upon a certificate of the Secretary-Treasurer of the Local Union that such fees, dues, fines and or assessments were levied in accordance with the Local Union's By-Laws.
e) The Union shall forward all authorization forms to the Employer. It shall be the responsibility of the Employer to take proper and due care of all authorization forms sent to the Employer by the Union.
f) At the beginning of each year, the Employer shall provide a schedule to all employees detailing the pay periods in which Union dues shall be deducted.

3.02 Fifteen (15) days after an employee commences employment, they shall have an amount equivalent to the monthly dues of the Union deducted from their wages, and that amount, along with the employee’s name, will be added to the Union check-off, before same is mailed to the Union. The Employer will deduct the Initiation Fee from the employee starting the first month of employment and prior to the employee completing sixty (60) calendar days of employment. The deduction of the Initiation Fee shall be in increments of fifty dollars ($50.00).
3.03 The Union shall forward all authorization forms to the Employer. It shall be the responsibility of the Employer to take proper and due care of all authorization forms sent certified to the Employer by the Secretary/Treasurer of the Union.
Section 2. The Employer shall remit each month to the Union, the amount of deductions made for that particular month including initiation fees, reinstatement fees, membership dues, and arrears, together with a list of employees with their social security numbers, hourly rate of pay, and arrearages per week/month, for whom such deductions have been made. The list will indicate all official personnel actions that result in a change in status of bargaining unit members, including new hires, terminations, leaves of absence, and layoffs. The remittance shall be forwarded not later than the 25th of the month following the month in which deductions are made. The Parties agree that they shall continue to meet and confer regarding the implementation of methods and processes that will improve the efficiency of compiling and transmitting information relevant to such deductions, including doing so electronically if possible.
Section 3. The Employer's obligation is limited solely to making the authorized deduction and such obligation shall cease at the time the employee is terminated or laid off for lack of work, including summer layoffs.
Section 4. The Union shall hold harmless the Employer from any and all claims that may arise out of the Employer’s compliance with this Article.
Section 5. The Employer shall deduct monthly or weekly a flat dollar amount, if the Employer’s payroll system permits, from the gross wages or salary of each employee who voluntarily executes the Committee on Political Education (COPE) payroll deduction authorization form that is Appendix C to this Agreement the contributions so authorized on that form, and remit those contributions to the Union at the same time that the Employer remits to the Union the Union dues that are separately voluntarily authorized by employees to be deducted from their gross wages or salaries and remitted to the Union pursuant to Article 2, Section 2 of this Agreement. The Employer may remit COPE contributions and Union dues to the Union by a single check, or by separate checks. With each COPE contribution remittance, the Employer shall provide the Union with a written itemization setting forth as to each contributing employee his or her name, Social Security number and total contribution amount. The parties acknowledge that the Employer’s costs of administration of this COPE payroll deduction have been taken into account by the parties in their negotiation of this Agreement and have been incorporated in the wage, salary, and benefits provisions of this Agreement. The Employer's responsibility under this Section is limited solely to disbursing the funds to the Union as provided in this Section. The Union shall assume all responsibility for distribution of the COPE contribution remittance to the COPE's specified on the form that is Appendix C.
