Description of Project EXHIBIT A SCOPE OF SERVICESIn accordance with the provisions of this grant agreement and all rules and regulations pertaining thereto, GRANTEE shall use the funds as follows:
Description of Project EXHIBIT A SCOPE OF SERVICESIn accordance with the provisions of the AGREEMENT and all rules and regulations pertaining thereto, GRANTEE shall use the Grant Award as follows: GRANTEE will partner with Abode Services to provide housing placement, intensive case management and housing retention services to homeless individuals identified as eligible for rapid re-housing at the Plaza Hotel in San Xxxx. The target population will be single adults experiencing homelessness who need short term assistance to achieve self- sufficiency. GRANTEE will also have a case manager focused on employment services and linkage to wraparound supportive services to increase income for the individuals served. Eligible participants under this AGREEMENT shall be all low-income, homeless individuals and families whose household income does not exceed 80% of area median income (AMI”) per the current California Department of Housing and Community Development (HCD) income limits residing in the Project Area. Eligible participantsare homeless individuals, homeless families, and homeless youth referred from Santa Xxxxx County’s Continuum of Care (“CoC”) Coordinated Assessment System. GRANTEE shall document each participant’s eligibility on intake sheets, which shall include verification of homelessness, information about participant’s ’s family size, total household income, gender of head of household, race and ethnic data. GRANTEE will receive participant referrals from CITY-designated rental assistance providers in the CITY’s Rapid Rehousing Program as well as receive new households from the community queue of eligible households generated from Homeless Management Information System (“HMIS”) in accordance with the CoC Coordinated Assessment System. Additionally, case management will include post-housing placement follow-up at six (6) and twelve (12) months after a participant has been placed into permanent housing. “Project Area” is the incorporated area of the City of San Xxxx.

  • Attachment A, Scope of Services The scope of services is amended as follows:

  • Detailed Description of Services / Statement of Work Describe fully the services that Contractor will provide, or add and attach Exhibit B to this Agreement.

  • SERVICE LEVEL DESCRIPTION The Fund Accounting Agreement is hereby amended by deleting the Service Level Description attached thereto and replacing it in its entirety with the Service Level Description attached hereto.

  • Project Description In two or three brief sentences, provide a concise description of your exhibition. Include the subject matter, type of objects to be included (paintings, sculpture, manuscripts, etc.), those responsible for organizing the exhibition, and catalogue author(s).

  • Description of Projects Services a. Project/Services to be performed by A-E shall consist of the work as specified herein and as required in Attachment A. If in the event Attachment A shall be in conflict with any provision of this Contract, the wording as set forth in Attachment A shall prevail. b. A-E shall be responsible for submitting all Projects/Services to County in a form which has been thoroughly reviewed and checked for completeness, accuracy and consistency by the registered professional named in Section 1.1.2 herein; and, any Projects/Services not meeting this requirement will be returned to A-E prior to review by County.

  • DESCRIPTION OF PROJECT For the Company to be eligible to earn the Maximum Credit Amount, the Company will satisfy its obligations as reflected in the following representations, which the IEDC has relied upon: A. The Company will complete the Project at the Project Location. B. The Company represents that the number of permanent, Full-Time Employees (as defined in Indiana Code § 6–3.1–13–4) from whom Indiana state income tax withholdings are retained by the State of Indiana, employed as of the Commencement Date at the Project Location, is the Base Employment Number. C. The Project will result in the creation of New Employees (as defined in Indiana Code § 6- 3.1-13-6) at the Project Location of at least the Additional Jobs Commitment. D. The average of the hourly wages, before benefits, paid to New Employees at the Project Location, will at least equal the Average Wage Commitment. E. At the discretion of the IEDC, New Employees that are paid an average wage of less than the Minimum Wage Commitment may be excluded for the purpose of calculating the credit amount. F. The Project is anticipated to involve at least the Capital Investment Amount.

  • Contract Exhibit J Quarterly Sales Report If a conflict exists among any of the Contract documents, the documents shall have priority in the order listed below: a) The Contract b) Statement of Work, Contract Exhibit A c) Additional Special Contract Conditions, Contract Exhibit D d) Special Contract Conditions, Contract Exhibit C e) Resume Acknowledgement Form, Contract Exhibit G f) Contractor Selection Justification Form, Contract Exhibit H

  • Site Description {Buyer Comment: Provide a legal description of the Site, including the Site map.}

  • Service Descriptions Credit Card processing services: Global Direct’s actions to the appropriate card associations and/or issuers (e.g., Visa, MasterCard, Diners, Discover); settlement; dispute resolution with cardholders’ banks; and transaction-related reporting, statements and products. Debit/ATM Processing Services: Global Direct has connected to the following debit card networks (“Networks”): Accel, AFFN, Interlink, MAC, Maestro, NYCE, Pulse, Star, and Tyme. Global Direct will provide Merchant with the ability to access the Networks that Global Direct has connected to for the purpose of authorizing debit card transactions at the point of sale from cards issued by the members of the respective Networks. Global Direct will provide connection to such Networks, terminal applications, settlement and reporting activities. EBT Transaction Processing Services: Global Direct offers electronic interfaces to Electronic Benefits Transfer (“EBT”) networks for the processing of cash payments or credits to or for the benefit of benefit recipients (“Recipients”). Global Direct will provide settlement and switching services for various Point of Sale transactions initiated through Merchant for the authorization of the issuance of the United States Department of Agriculture, Food and Nutrition Services (“FNS”) food stamp benefits (“FS Benefits”) and/or government delivered cash assistance benefits (“Cash Benefits, ”with FS Benefits, “Benefits”) to Recipients through the use of a state-issued card (“EBT Card”). With respect to Visa and MasterCard products, Merchant agrees to pay and Merchant's account(s) will be charged pursuant to Section 5 of this Agreement for any additional fees incurred as a result of Merchant's subsequent acceptance of transactions with any Visa or MasterCard product that it has not elected to accept.
