Discharge Services. The process to prepare the Client for referral into another level of care, 11 post treatment return or reentry into the community, and/or the linkage of the individual to essential 12 community treatment, housing and human services. CONTRACTOR shall provide or arrange for 13 transportation of Clients to aftercare destination. CONTRACTOR shall begin discharge planning 14 immediately after enrollment. CONTRACTOR shall develop a formal discharge plan with the Client no 15 later than fourteen (14) calendar days prior to Client’s successful completion of the program. The exit 16 plan shall be completed and signed by CONTRACTOR staff and Client. The exit plan shall be 17 documented in the Client’s chart.

34 a. Discharge Plan – CONTRACTOR shall develop a formal discharge plan within thirty (30) 35 calendar days prior to Client’s planned discharge from the program. A discharge plan is to be 36 completed for each Client, except a Client with whom the provider loses contact. The discharge plan 37 shall be completed and signed by the LPHA or counselor and the Client. A copy of the discharge plan 1 shall be provided to the Client and retained in the Client’s record. The discharge plan shall include, but 2 not be limited to, all of the following:
3 1) A description of each of the Client’s relapse triggers;
4 2) A plan to assist the Client to avoid relapse when confronted with each trigger;
5 3) A support plan; and
6 4) Referrals and linkages to appropriate resources.

29 2) Discharge Summary – The discharge summary is to be completed by the LPHA or 30 counselor within thirty (30) calendar days of the date of the last face-to-face treatment contact with the 31 Client. The discharge summary shall include all of the following:
32 i) The duration of the Client’s treatment as determined by the dates of admission 33 to and discharge from treatment;
34 ii) The reason for discharge;
35 iii) A narrative of the treatment episode; and

Recovery Services – Recovery Services are designed to support recovery and prevent 2 relapse with the objective of restoring the Client to their best possible functional level. Recovery 3 Services emphasize the Client’s central role in managing their health, use effective self-management 4 support strategies, and organize internal and community resources to provide ongoing self-management 5 support to beneficiaries. Clients may receive Recovery Services based on self-assessment or provider 6 assessment of relapse risk. Clients do not need to be diagnosed as being in remission to access Recovery 7 Services. Clients may receive Recovery Services while receiving MAT services, including NTP 8 services. Clients may receive Recovery Services immediately after incarceration with a prior diagnosis 9 of SUD. Discharge Services. A variety of discharge services may be desired such as e-scripts to the Hospital owned or local pharmacies, e-ordering home care services, and messaging to the patient’s message board regarding the date.

25 1) The duration of the Client’s treatment as determined by the dates of admission and
27 2) The reason for discharge, including whether the discharge was voluntary or 28 involuntary and whether the client successfully completed the program;
29 3) A narrative summary of the treatment episode;
30 4) The Client’s prognosis;
31 5) Current alcohol and/or drug usage;
32 6) Vocational and educational achievements;
33 7) Legal status;
34 8) Linkages and referrals made;
35 9) Clients comments; and
36 10) A description of the Client’s goals and achievement towards those goals as 37 described in the Client’s treatment plan.
Discharge Services this customized option will be charged based on the nature and extent of the request.

Discharge Services. The contractor shall connect youth and their families with resources and referral information to assist in meeting the needs of individuals. This includes securing all necessary records and planning with clinical staff and other support staff to ensure continuity of care after the child is discharged.
