Disclosure of Tobacco Industry Funding. The Grantee shall report annually to the Commission the receipt of any moneys from a person, persons, firm, partnership, corporation, or combination thereof that derives more than 15 percent of its revenue from the production or sales of tobacco products. The report shall include the name of the entity, the amount received, the percentage of the agency’s total budget the amount represents, the purpose of the contribution, and any efforts made to obtain alternative funding.

Disclosure of Tobacco Industry Funding. On November 12, 2004, the Commission amended the Comprehensive Tobacco Control Policy to require all organizations that are in compliance with the policy areas listed above to implement and conduct one tobacco health education training for their staff and/or clients during the funded term. Contractors will be required to evaluate the effectiveness of the training by conducting a pre and post-test as well as a general training evaluation on the effectiveness of the training. Contractor will be required to submit a summary of the evaluations to First 5. Contractor is required to attend, during the grant year, a 3-hour training focused on tobacco control policy/education sponsored by the Commission. Furthermore, Contractor will show a good faith effort during the grant term in the following three areas:
