Faculty Observation Report. The Faculty Observation Report Form corresponds to a list of behaviors associated with instructional performance. Faculty observations are to be conducted by the faculty member’s Department Head or Program Director, colleagues, and Associate Xxxx for Faculty Evaluation and Hiring, as appropriate (see additional information below). Instructional activities to be observed include but may not be limited to classroom instruction, laboratory and clinical instruction, and web and web-based instruction. Annual Faculty Performance Evaluations are to include reflection on instructional activities as recorded on the Faculty Observation Report. Faculty members will offer their evaluators at least three different class periods from which the evaluator can choose to come and observe. Full-time faculty classroom observations will be conducted as follows: Pre-tenure  A tenure-track faculty member will be observed once per semester.  Four of those observations will be performed by the Associate Xxxx for Faculty Evaluation and Hiring, and four of those observations will be performed by the appropriate Department Head/Program Director or peer. Pre-Associate Professor  A tenured faculty member who has not achieved the rank of Associate Professor will be observed once per year.  Two of those observations will be performed by the Associate Xxxx for Faculty Evaluation and Hiring, and two of those observations will be performed by the appropriate Department Head/Program Director or peer.  A faculty member at this level also will seek yearly written feedback on an element of his or her teaching (syllabus, assignment, assessment, classroom policies and practices, teaching technique, etc.) from a peer.

  • Informal Observation Additional evaluation of employees may occur through informal observations by the Employer. It may not be necessary to reduce such evaluations to writing, and a verbal discussion between the Employer and the employee concerning the informal observations may suffice. If a written evaluation is deemed necessary, Procedures C and D as outlined above shall be followed.

  • Pre-Observation Conference The pre-observation conference shall be held prior to each formal observation. The teacher and evaluator will mutually agree when to conference. The purpose of the pre-observation conference is to discuss the employee’s goals, establish a date for the formal observation, and to discuss such matters as the professional activities to be observed, their content, objectives, strategies, and possible observable evidence to meet the scoring criteria.

  • Evaluation Reports Where a formal evaluation of an employee's performance is carried out, the employee shall be provided with a copy to read and review. Provision shall be made on the evaluation form for an employee to sign it. The form shall provide for the employee's signature in two (2) places, one indicating that the employee has read and accepts the evaluation, and the other indicating that the employee disagrees with the evaluation. The employee shall sign in one of the places provided within seven (7) calendar days. No employee may initiate a grievance regarding the contents of an evaluation report unless the signature indicates disagreement with the evaluation. The employee shall receive a copy of the evaluation report at the time of signing. An evaluation report shall not be changed after an employee has signed it, without the knowledge of the employee, and any such changes shall be subject to the grievance procedure.

  • Post-Observation Conference 1. The post-observation conference between the evaluator and teacher will be held no later than five (5) days after the formal observation.

  • Evaluation Report 20.5.4.1 The summary evaluation report shall be prepared by the Faculty Evaluation Committee and administrative evaluator(s) and shall include each evaluator’s individual rating. The summary evaluation report shall take into account the results of each of the evaluation components (Section 20.5.3) in order to arrive at an overall rating. When the committee and the administrative evaluator(s) cannot reach an agreement as to the overall rating, the report must include written explanation.

  • Inspection Reports A "full inspection report" comprises a Site Master File (compiled by the manufacturer or by the inspectorate) and a narrative report by the inspectorate. A "detailed report" responds to specific queries about a firm by the other Party.

  • Observation Conference 1. All formal observations shall be preceded by a conference between the evaluator and the teacher in order for the teacher to explain plans and objectives for the work situation to be observed.
