Family Responsibility Leave. In the event of a medical or dental appointment which the employee has not been able to schedule during non-working hours and/or a serious illness or injury occurring to/for an employee’s spouse, parent, or child, the employee may utilize up to five (5) days or forty (40) hours of accumulated sick leave entitlement per calendar year to attend to such family responsibilities. The Employer may request and if so, shall be provided with verification of the appointment, certificate of illness or injury and the time involved in order to establish eligibility under this article.
