Functionality Overview. The Current Child Placement Agreement tab provides users the ability to quickly and easily view the current Child Placement Agreement details without having to review various tabs of information. Following the initial creation of the Child Placement Agreement page, each time the page is subsequently launched, it defaults to the first tab, Current Child Placement Agreement. This tab contains the following fields: Purpose, Type, Effective Date, and Provider Name. These fields display for the most recently inserted record based on the effective date documented on the Child Placement Agreement Details tab. Reasons for Agreement, Placement Requirements, Placement Requirements Description, and Caregiver Supports group boxes are pre-filled based on logic related to the most recent Effective Date row inserted on the Child Placement Agreement Details tab, as defined in the table directly below.
Functionality Overview. The Child Placement Agreement Details tab provides the ability to document Agreement Details, as well as Termination Information. It allows the user the ability to capture the main agreement details such as the reasons the agreement is needed, access the related template to provide to the Caregiver, upload the signed agreement, and document the termination of the agreement at such time it is no longer needed based on policy and practice. The Child Placement Agreement Details tab contains two group boxes: Agreement Details and Agreement Termination. The Agreement Details group box contains a repeating table with an Insert button to insert a new row, as needed. Upon initial creation of the Child Placement Agreement page, the first row will automatically be populated and at least one row must be documented in order to save the Child Placement Agreement page. Each time a row is inserted on the Agreement Details tab, after clicking the Add Reasons hyperlink, the Add Reasons pop-up page displays pre-filled from the previous Add Reasons pop-up page; however, it will be user modifiable. The repeating group box contains the following columns: Purpose, Type, Effective Date, Reason(s), Provider, Agreement, Signed Agreement, and Actions. For the row with the radio button selected, a narrative Comments box will allow the user to document information specific to the row selected. At such time that neither a Behavior Management Plan nor Care Precautions are needed, the Child Placement Agreement can be terminated. To Terminate the Agreement, the user selects the Reason Plan is no longer required, enters the Terminate Date, and enters text into the associated narrative text field. Note that upon terminating the Agreement, the page will become frozen.
Functionality Overview. The Add Reasons Pop-Up page provides the ability for workers to document the reasons that the Child Placement Agreement is being created and to ensure that all caregivers are made aware of a child’s behaviors or cirucmstances that need to be addressed. To access the Add Reasons Pop-Up page, users click on the Add Reasons hyperlink located in the Reasons column on the Child Placement Agreement Details tab. Each time a row is inserted on the Agreement Details tab, upon clicking the Add Reasons hyperlink, the Add Reasons pop-up page pre-fills from the previous Add Reasons pop-up page and is user modifiable. In order to successfully save the Add Reasons Pop-Up page, at least one category with an associated sub-category must be selected and the associated narrative documented. Note that Human Trafficking is it’s own category with no associated sub- category(ies). Once a category is selected, it’s associated sub-category(ies) are enabled and the selection of at least one is required (unless it is the category of Human Trafficking that has no sub- category(ies)). If the ‘Other’ sub-category is selected, the associated text field is then enabled and required. Upon clicking Continue to return to the Child Placement Agreement page and successfully saving, the Add Reasons hyperlink dynamically changes to display as ‘View Reasons’ and remains a hyperlink. Clicking the View Reasons hyperlink launches the Add Reasons pop-up page in Edit mode up until the overall Child Placement Agreement page is terminated at which point the page as a whole and the associated pop-up page becomes disabled and grayed out.
