Group Accidental Death and Dismemberment Insurance Plan. The Company agrees that the Group Accidental Death and Dismemberment Insurance Plan, which is covered by Supplementary Agreement, shall be continued for the life of this Agreement.

Group Accidental Death and Dismemberment Insurance Plan. It is agreed that for the term of the labor agreement the Company will provide Basic Accidental Death and Dismemberment Insurance in the amount of two times the employee’s annual base wage (maximum of $60,000) at no cost to the employee for all full-time regular employees. In addition, the company agrees to offer opportunities for the employee to elect additional AD&D coverage for themselves and certain eligible family members. The cost for the additional coverage is subject to change, and these changes will be discussed with the Union prior to implementation. The Seat Belt Incentive Program is eliminated. These changes will be implemented and effective on January 1, 2008. This language supersedes all previous agreements and prospectively constitutes the entire agreement relative to the Accidental Death & Dismemberment Insurance Plan.

  • Group Life and Accidental Death and Dismemberment (a) The Employer will pay 100% of the premiums for the group life and accidental death and dismemberment insurance plans.

  • Basic Life and Accidental Death and Dismemberment Coverage The Employer agrees to provide and pay for the following term life coverage and accidental death and dismemberment coverage for all supervisors eligible for an Employer Contribution, as described in Section 3. Any premium paid by the State in excess of fifty thousand dollars ($50,000) coverage is subject to a tax liability in accord with Internal Revenue Service regulations.   • Accidental Death and Dismemberment The Employer agrees to provide all active full-time employees with Accidental Death and Dismemberment benefit coverage equal to one (1) times their annual earnings in case of accidental death. Coverage is also provided for other losses such as speech and hearing, use of arms and legs, etc.

  • Long Term Disability Insurance Plan The Employer shall provide a mutually acceptable long-term disability insurance plan, a copy of which shall appear in Appendix “A” – Long-Term Disability Insurance Plan. The plan shall provide post-probationary regular employees with salary continuation as per Appendix “A” until age sixty-five (65) in the event of a disability. The cost of the plan shall be borne by the Employer.

  • Optional Life and Disability Coverages In order for coverage to become effective, the employee must be in active payroll status and not using sick leave on the first day following approval by the insurance company. If it is an open enrollment period, coverage may be applied for but will not become effective until the first day of the employee's return to work.
