Major Restorative Procedures: Dentures, Partial Plates, Crowns and Caps - on a 50/50 co-insurance basis within the existing $2,000.00 annual maximum. Orthodontic coverage to a maximum of $1000.00 per person under the age of 18 years, once per lifetime. (ix) Long Term Disability Plan, to include: after 26 weeks, 2/3 of earnings to a maximum of $1,550.00 per month, to age 65, but is not retroactive.
