Accidental Death Insurance. The State shall provide a State-sponsored Accidental Death Insurance Plan which has a benefit of $100,000 in case of an employee's accidental death in line of duty.
Accidental Death Insurance. The accidental death insurance policy the County maintains for employees covered by this Agreement shall be payable in the amount of fifty thousand dollars ($50,000) to an employee’s designated beneficiary for death or personal loss caused by an accident on or off the job.
Accidental Death Insurance. In addition to any other life insurance or death benefit provided by the County, the County shall pay a death benefit of ten thousand dollars ($10,000.00) upon the death of any employee covered by this Agreement whose death results from an accident on or off the job.

Death must result directly from an Accident, and independently from any other cause, be sustained during an Insured trip and occur within 365 days following the date of the Accident.

Accidental Death Insurance. An Engineer's coverage provided by the Company is $50,000. Employees requiring assistance concerning the foregoing may contact a Human Resources Representative for further information. In addition, the BLET National Plan Accidental death benefit of $16,000 will also apply.
Accidental Death Insurance. Pursuant to the provisions of MGL c32B, s11D, the City shall provide and pay seventy- five (75%) percent of the premium cost, for service-connected group accidental death insurance coverage in an amount equal to twice the maximum amount to which each eligible employee would be entitled if insured for the maximum amount of group accidental death and dismemberment insurance provided under MGL c32B, s11A. It is understood that the City may decide to be a self-insurer. In the absence of an effective insurance policy, the City shall be deemed a self-insurer hereunder.
Accidental Death Insurance. The Insured Person is hereby covered for the Accidental loss of life in the amount of CAN $25,000. Death must result directly from an Accident, and independently from any other If the attending Physician determines and confirms in writing that as a result of an Emergency, You are incapable of driving Your Vehicle to Your residence in Your prov- ince of residence or to the nearest appropriate rental agency, the Insurer will reimburse the lesser of the following actual reasonable and necessary costs incurred to return Your Vehicle, up to a maximum of CAN $2,000.
1) The cost of a one-way economy class plane ticket, gasoline, meals (except alcohol) and overnight commercial accommodation incurred by one individual, or
2) The cost of the return performed by a professional agency. These expenses will be refunded only if Your Vehicle is returned to Your residence in Your province of residence within 30 days following Your own return. Original detailed receipts will be requested. Car rental costs while awaiting the return of Your Vehicle are not covered.
Accidental Death Insurance. If you die as a direct result, and from no other cause, of a Covered Accident within 365 days from the date of a Covered Accident, we will pay the Principal Sum. Refer to the Accidental Death Insurance Guide to Benefits for complete coverage description and exclusions. $25,000 death benefit per eligible account holder. The benefit provides roadside service for up to $80 per occurrence limit. There will be one occurrence for the same service during any continuous seven-day period and a maximum of four services per twelve-month period. Covered services include towing assistance, battery service, flat tire assistance, fuel, oil, fluid, water delivery, lock-assistance, collision assistance and extrication assistance. The roadside assistance is offered through Road America Motor Club. Refer to the Roadside Assistance Terms and Conditions at xxx.XxXxxx.xxx for a complete list of terms and conditions. This is not a Roadside Assistance reimbursement service.

(c) Orthodontia ) 60% coverage to a maximum of $2000.00 per listed dependent per lifetime. An employee will be eligible for all coverage outlined in (a) and (b) on the first (1st) day of the month following his commencement of employment. New employees shall be eligible for Dental Coverage (c) on the first day of the third month of employment. The Company will provide and maintain coverage as set out in Great West Life Assurance Company Policy #102300 and the RBC Insurance Co. These benefits shall remain regardless of whether Thermo King of BC changes plan carriers. The parties further agree that the employer will provide an employee-paid long term disability plan for those eligible. The provisions of coverage will be consistent with and set out in Great West Life Assurance Company Policy #102300 dated June 01, 2001. These benefits shall remain regardless of whether Thermo King of BC changes plan carriers. The Company will provide insurance coverage allowing for one pair of eyeglasses or contact lenses up to two hundred dollars ($200.00) in a twenty-four (24) month period. This coverage is for employee and family members. Commencing October 1st, 2014, this amount shall increase to two dollars and sixty-five cents ($2.65). The Operating Engineers' Benefits and Pension Plans shall be controlled by a Board of Trustees composed of eight (8) representatives designated by the Union. The Employer agrees to be bound by the terms of the Trust Agreement. The Employer is required to report on the forms provided by the Benefits and Pension Plans. Contributions must be forwarded by the Employer to the Operating Engineers' Benefits and Pension Plans by the fifteenth (15th) day of the month following that which contributions cover. In the event an Employer fails to remit contributions to this Plan, in conformity with this section of the Agreement, the Union is free to take any economic action it deems necessary against such Employer, and such action shall not be considered a violation of this Agreement.
